慢性心力衰竭患者血浆中IL-27和IL-18水平变化及其临床意义

摘　　要：目的探讨慢性心力衰竭(CHF)患者血清白细胞介素(IL)-27、白细胞介素(IL)-18、超敏C反应蛋白(hs-CRP)的水平变化情况及与左室射血分数(LVEF)的关系。方法选取2013年12月至2014年8月于广东省第二人民医院心血管内科住院的CHF患者60例,按照美国纽约心脏病学会(NYHA)心功能分级分为NYHA II级组(18例)、NYHA III级组(22例)、NYHA IV级组(20例),同期我院健康体检者30例作为对照研究对象,分别检测CHF组及对照组的血清IL-27和IL-18(酶联免疫吸附法)、hs CRP(免疫比浊法)水平。所有入选者均进行超声心功能检测。结果 CHF患者血清IL-27、IL-18、hs-CRP水平较对照组升高(P <0. 05),且随着心功能分级升高,增加越明显。IL-27分别与IL-18、hs-CRP呈正相关(P <0. 05); IL-27、IL-18、hs-CRP与LVEF呈负相关(P <0. 05)。结论 IL-27和IL-18与心衰的发生、发展密切相关。Objective To investigate the relationship of serum IL-18,IL-27,hs-CRP and left ventricular ejection fraction(LVEF) in chronic heart failure(CHF).Methods 60 patients with heart failure admitted in our department were divided into NYHA Ⅱ group,NYHA Ⅲ group and NYHA Ⅳ group according to New York Heart Association(NYHA) functional classification.30 normal people verified with medical examination were selected as the control group.The concentrations of plasma IL-27,IL-18 and high-sensitivity C-reactive protein(hsCRP) were measured by enzyme-linked immunosorbent assay(ELISA),immunoturbidimetric assay,respectively.Heart function was measured by ultrasound cardiography.Results The levels of IL-18、IL-27 and hsCRP in CHF were significantly higher than the control group,and the increase was in direct proportion to that in New York Heart Association(NYHA)functional classification.IL-27 levels were in direct proportion to IL-18 and hsCRP.And IL-18 levels were in direct proportion to hsCRP.IL-27、IL18、hsCRP levels were in inverse proportion to LVEF.Conclusion IL-27 and IL-18 has a closely correlation with the development of CHF.
