摘　　要：目的探讨症状性筛查胃癌的重要性,分析胃癌的临床特点,总结诊断经验并提高早期胃癌的检出率。方法对我院内镜中心2006年1月至2018年10月接受胃镜检查的25万例症状人群的临床资料进行回顾性分析。结果确诊早期胃癌283例,进展期胃癌1 157例。所确诊胃癌患者中,年龄主要分布在40～60岁和60～80岁,分别占26. 4%和62. 2%,男性占75. 0%,女性占25. 0%,主要症状为上腹部隐痛和食欲减退,分别占60. 0%和40. 0%,病理类型中,以低分化腺癌及高级别上皮内瘤变多见,各占45. 0%和18. 1%。病变部位以胃窦部、胃体多见,分别占38. 2%、37. 8%。结论早期胃癌以男性多见,低分化腺癌占比高,好发部位为胃窦部。内镜医师在日常工作中,对目标人群应行胃镜筛查,提高早期胃癌诊治率。Objective To explore the importance of symptomatic screening for gastric cancer and analyze the clinicopathological features.Meanwhile,to improve the detection rate of early gastric cancer.Methods From Jan.2006 to Oct.2018,250 thousands patients presented with upper abdominal pain,discomfort and distention were screened for gastric cancer in the Endoscopy Center of Minhang Hospital,Fudan University.Results All of 1 440 gastric cancer cases were confirmed in the study.Among these patients,283 cases were diagnosed as early gastric cancer,while 1 157 cases were as advanced gastric cancer.Among the patients with confirmed gastric cancer,the age distribution was mainly ranged from 40 to 60 years old and 60 to 80 years old,accounting for 26.4%and 62.2%,respectively,male patients accounted for 75.0%,and female patients accounted for 25.0%.The main symptoms were upper abdominal pain(60.0%)and loss of appetite(40.0%).The main pathological types were poorly differentiated adenocarcinoma and high-grade intraepithelial neoplasia,accounting for 45.0%and 18.1%.The lesions were more common in the antrum and corpus,accounting for 38.2%and 37.8%,respectively.Conclusion The gastric cancer is more likely to find in men,gastric antrum is high incidence and the proportion of poorly differentiated adenocarcinoma is higher than other types.Endoscopic doctors should perform gastroscopy screening on the target population in daily work to improve the diagnosis and treatment rate of gastric cancer.
