不同性别双相障碍患者临床特征对比分析

摘　　要：目的探讨不同性别双相障碍患者临床特征的差异性。方法选取816例首次确诊的双相障碍患者,按性别不同分为男性组(409例)和女性组(407例),对比两组在病程、家族史、首发诊断、伴发精神病性症状、精神刺激史等方面的异同点。结果男性患者受教育年限长于女性患者(P <0. 01),女性已婚患者多于男性已婚患者(P <0. 01)。女性患者以抑郁为首发多于男性患者,男性患者以躁狂为首发多于女性患者(P <0. 01);女性患者伴有精神刺激史多于男性患者(P <0. 05);双相障碍患者多以慢性起病为主,其中女性患者以急性、亚急性起病为主多于男性患者,男性患者以慢性起病为主多于女性患者(P <0. 01);女性患者自杀史多于男性患者(P <0. 01);女性患者伴发精神病性症状多于男性患者(P <0. 01)。女性患者误诊率高于男性患者(P <0. 01)。结论不同性别的双相障碍患者具有不同的临床特征,在临床工作中应对此加以区别对待。Objective To explore the differences of clinical features in patients with bipolar disorder between different genders.Methods A total of 816 patients with bipolar disorder diagnosed for the first time were divided into male group(409 cases)and female group(407 cases),and their disease course,family history,initial diagnosis,accompanied psychiatric symptoms and history of mental stimulation were compared.Results Male patients got education for longer time than that in female patients(P<0.01).There were more married female patients than married male patients(P<0.01).The female patients with depression as the first onset were more than male patients and the male patients with mania as the firs onset were more than female patients(P<0.01).The female patients with mental stimulation history,suicide history and accompanied psychiatric symptoms were more than male patients(P<0.05).The patients with bipolar disorder were mainly chronic onset,in which female patients with acute and subacute onset were more than male patients and male patients with chronic onset were more than female patients(P<0.01).The misdiagnosis rate in female patients was significantly higher than that in male patients(P<0.01).Conclusion There are different clinical features in patients with bipolar disorder between different genders,which should be treated differently in clinical work.
