摘  要:目的比较二氧化锆固位钉与金属固位钉在离体牙上的拉伸固位强度、抗剪切强度、三点弯曲强度的差异;观察二氧化锆固位钉在≥65岁老年病人前牙牙体缺损修复中的临床疗效。方法 (1)选取老年病人因牙周病拔除的离体牙60颗,分别采用二氧化锆固位钉与金属固位钉配合纳米树脂充填修复,并对其进行力学性能测试。(2)随机选取年龄≥65岁,前牙大面积缺损的老年病人168例,随机分为A、B、C 3组。A组采用金属牙本质钉加复合树脂联合修复,B组采用直接复合树脂修复,C组采用二氧化锆固位钉加复合树脂联合修复。1年后复查,比较3组临床疗效。结果 (1)二氧化锆固位钉组的抗拉伸强度、抗剪切强度高于金属固位钉,三点弯曲强度低于金属固位钉,差异均有统计学意义(P<0. 05)。(2) 1年后复查发现,C组成功率为89. 7%,B组成功率为79. 2%,A组成功率为65. 5%,3组间比较差异有统计学意义(P<0. 05)。结论 (1)二氧化锆固位钉联合树脂对离体牙的黏结能力强于金属固位钉,金属固位钉的机械强度高于二氧化锆固位钉。(2)二氧化锆固位钉加复合树脂联合修复老年前牙牙体缺损更有优势,可提高治疗成功率,值得在临床进一步推广应用。Objective To compare the difference of tensile bond strength,shear bond strength and flexural strength between zirconia ceramic pin and metal pin,and to evaluate the clinical effects of zirconia ceramic pin on restoration of anterior teeth in the elderly patients aged 65 years and over.Methods(1)Sixty isolated teeth of the elderly patients with periodontal disease were selected and repaired with zirconia ceramic pin and metal pin respectively.The strength of tensile bond,shear bond and flexural were tested.(2)A total of 168 elderly patients with anterior teeth defect were selected,and were divided into group A,group B and group C randomly.Group A was treated with metal pin in combination with composite resin;Group B was treated with composite resin directly.Group C was treated with zirconia ceramic pin in combination with composite resin.All the cases were followed up for one year to evaluate the clinical effects.Results(1)The tensile bond strength and shear bond strength of the zirconia ceramic pin were stronger than those of the metal pin,and the flexural strength of the metal pin was higher than that of the zirconia ceramic pin(P<0.05).(2)After one year of follow-up,the successful rate of group A was 65.5%,compared with 79.2%in group B,89.6%in group C,and the differences were statistically significant(P<0.05).Conclusions Zirconia ceramic pin combined with resin has stronger adhesion to the isolated teeth than the metal pin,and the mechanical strength of the metal pin is higher than that of the zirconia ceramic pin.Zirconia ceramic pin combined with resin has more advantages in restoring the anterior teeth of elderly patients,which can significantly enhance the successful rate of the treatment.
