SPECT/CT显像对恶性肿瘤患者骨骼单发病灶定性诊断的临床应用价值

摘　　要：目的观察SPECT/CT显像对恶性肿瘤患者骨髂单发病灶定性诊断的临床应用价值。方法选取我院2016年10月～2017年8月收治的50例恶性肿瘤患者,实行回顾性分析。所有患者均在骨髂单发病灶定性诊断中采用SPECT/CT检查。观察融合显像诊断符合率、融合显像诊断情况分析、融合显像增益情况分析。结果 50例患者病灶主要发生在脊柱、肋骨、胸骨、盆骨、四肢等位置。全身平面骨显像、局部CT成像、SPECT/CT融合显像诊断符合率分别为64.0%、70.0%、96.0%,组间比较,差异有统计学意义(P<0.05)。全身平面骨显像、SPECT/CT融合图像定性病灶分别为32例、46例。其中,SPECT/CT融合显像可为全身骨平面显像诊断提供帮助。SPECT/CT灵敏度、特异度、阴性预测值、阳性预测值,与全身骨显像比较,差异有统计学意义(P<0.05)。结论 SPECT/CT显像对恶性肿瘤患者骨髂单发病灶定性诊断,具有重要的临床应用和推广价值。Objective To observe the clinical value of SPECT/CT imaging in the qualitative diagnosis of bone single lesion of patients with malignant tumors.Methods A retrospective analysis was performed on 50 patients with malignant tumors admitted in our hospital from October 2016 to August 2017.All patients underwent SPECT/CT examination in the qualitative diagnosis of bone single lesion.The fusion imaging diagnostic coincidence rate,fusion imaging diagnosis analysis,and fusion imaging gain analysis were observed.Results The lesions of 50 patients mainly occurred in the spine,ribs,sternum,pelvis and limbs.The coincidence rates of whole body planar bone imaging,local CT imaging,and SPECT/CT fusion imaging were 64.0%,70.0%,and 96.0%,respectively.The difference was statistically significant among three groups(P<0.05).Qualitative lesions of whole body planar bone imaging and SPECT/CT fusion images were 32 cases and 46 cases,respectively.Among them,SPECT/CT fusion imaging could help the diagnosis of whole body bone imaging.There was statistically significant difference in sensitivity,specificity,negative predictive value,positive predictive value,between SPECT/CT and whole body bone imaging(P<0.05).Conclusion SPECT/CT imaging has important clinical application and promotion value for the qualitative diagnosis of bone single lesion in patients with malignant tumors.
