腹腔镜直肠癌手术与传统开腹直肠癌手术的近远期疗效对比分析

摘　　要：目的对比腹腔镜直肠癌手术与传统开腹直肠癌手术的近远期疗效。方法 60例直肠癌患者,随机分为对照组和观察组,每组30例。对照组患者采用传统开腹直肠癌手术,观察组患者采用腹腔镜直肠癌手术,对比两组患者的近期疗效、远期疗效、并发症发生情况。结果观察组手术时间、术后排气时间、进固体食物时间、住院时间均短于对照组,术中出血量少于对照组,差异具有统计学意义(P<0.05)。观察组局部复发、远处转移、切口种植、切口疝等总发生率为30.00%,与对照组的33.33%对比差异无统计学意义(P>0.05)。观察组患者并发症发生率为16.67%,明显低于对照组的43.33%,差异具有统计学意义(P<0.05)。结论腹腔镜直肠癌手术的近期疗效及安全性优于传统开腹直肠癌手术,但两者远期疗效相当,临床可优先选择腹腔镜直肠癌手术。Objective To compare the short-term and long-term efficacy of laparoscopic rectal cancer surgery and traditional open rectal cancer surgery.Methods A total of 60 rectal cancer patients were randomly divided into control group and observation group,with 30 cases in each group.The control group was treated with traditional open rectal cancer surgery,and the observation group was treated with laparoscopic rectal cancer surgery.The short-term efficacy,long-term efficacy and occurrence of complications of the two groups were compared.Results The observation group had shorter operation time,postoperative exhaust time,solid food intake time,hospitalization time than the control group,and less intraoperative bleeding volume than the control group.Their difference was statistically significant(P<0.05).The observation group had no statistically significant difference in total incidence of local recurrence,distant metastasis,incision implantation and incisional hernia as 30.00%,comparing with 33.33%in the control group(P >0.05).The observation group had obviously lower incidence of complications as 16.67%than 43.33%in the control group,and the difference was statistically significant(P<0.05).Conclusion Laparoscopic rectal cancer surgery provides better short-term efficacy and safety than traditional open rectal cancer surgery,but the long-term efficacy of two methods is comparable.Laparoscopic rectal cancer surgery is preferred in clinic.
