手法碎核小切口非超乳技术在硬核白内障手术中的应用

摘　　要：目的对比研究手法碎核小切口白内障摘除术与超声乳化白内障摘除术治疗硬核性白内障的临床疗效。方法选取本院眼科2017年3月-2018年9月收治的46例(52眼)其中男18例(20眼)女28例(32眼)随机分为常规组(白内障超声乳化摘除术)和研究组(手法碎核小切口白内障非超乳摘除术)按照Emery分类法本组46例52眼均为Ⅳ-Ⅴ级核,随机分组,每组26眼,并观察两组患者术后1周和1月视力、角膜水肿和并发症情况。结果两组患者术后1天1周对比研究组(手法碎核小切口非超乳白内障摘除组)术后矫正视力>0.5占88.5%,角膜水肿轻、并发症少,明显优于常规组(白内障超声乳化摘除组)1月视力无明显差异。结论对于硬核白内障,手法碎核技术较常规超声乳化技术,操作更安全,视力恢复更快,角膜水肿发生率更低,值得临床推广使用。Objetive To compare the clinical efficacy of manual nuclear fragmentation small incision cataract extraction and phacoemulsification in the treatment of hard nucleus cataract.Methods From March 2017 to September 2018 in the ophthalmology department of our hospital,46 cases(52 eyes)comprised of 18 males(20 eyes)and 28 females(32 eyes)were randomly divided into conventional group(phacoemulsification cataract extraction group)and study group(manual nuclear fragmentation small incision nonphacoemulsification cataract extraction group),with 26 eyes in each group.According to Emery classification,these patients with cataract were classified as gradeⅣ-Ⅴnucleus.The visual acuity,corneal edema and complications of the two groups were observed in one week and one month after operation.Results The corrected visual acuity >0.5 accounted for 88.5%in the study group,corneal edema was mild and complications were less,which was significantly better than that of the conventional group 1 week after operation.There was no significant difference in visual acuity in 1 month after operation between two groups.Conclusion For hard nucleus cataract,manual nucleus fragmentation is safer than conventional phacoemulsification,and has an advantage of faster visual recovery and lower incidence of corneal edema,which is worthy of clinical application.
