术前新辅助化疗联合手术治疗Ⅰb2和Ⅱa期宫颈癌的临床效果

摘　　要：目的评价术前新辅助化疗(NACT)联合手术治疗Ⅰb2和Ⅱa期宫颈癌的临床效果。方法将我院收治的86例Ⅰb2和Ⅱa期宫颈癌患者根据术前是否行NACT分为直接手术组(43例,直接手术)和NACT组(43例,NACT+手术)。比较两组的临床疗效。结果 NACT组术前化疗总有效率为74.42%。NACT毒副反应均为轻度,患者均可耐受。化疗后,NACT组临床分期优于化疗前(P<0.05)。NACT组手术时间、术中出血量优于直接手术组,淋巴结转移、脉管受累、深肌层浸润及宫旁浸润发生率均低于直接手术组(P<0.05)。结论采用NACT联合手术治疗Ⅰb2和Ⅱa期宫颈癌效果显著,其可改善肿瘤分期及手术质量,且毒副反应小。Objective To evaluate the clinical effect of preoperative neoadjuvant chemotherapy(NACT)combined with surgery therapy for cervical cancer in stageⅠb2 andⅡa.Methods Eighty-six patients with cervical cancer in stageⅠb2 andⅡa admitted in our hospital were divided into direct surgery group(43 cases,direct surgery)and NACT group(43 cases,NACT+surgery)according to whether NACT was performed before surgery.The clinical effects of the two groups were compared.Results The total effective rate of preoperative chemotherapy in the NACT group was 74.42%.The toxicity and side effects of NACT were mild and tolerable for patients.After chemotherapy,the clinical stage of the NACT group was better than that before chemotherapy(P<0.05).The surgical time and intraoperative bleeding volume in the NACT group were better than those in the direct surgery group,and the incidences of lymphatic metastasis,vascular involvement,deep muscle infiltration and parametrial infiltration were lower than those in the direct surgery group(P<0.05).Conclusion NACT combined with surgery therapy for cervical cancer in stageⅠb2 andⅡa has a significant effect,which can improve the stage of cervical cancer and the quality of surgery,and it has slight toxicity and side effects.
