牙周炎合并T2DM患者血清糖脂代谢、炎性因子水平的变化及与牙周病变的关系

摘　　要：目的探讨牙周炎合并2型糖尿病(T2DM)患者血清糖脂代谢、血清炎性因子水平的变化及其临床意义。方法牙周炎伴T2DM患者40例(A组)、单纯T2DM患者40例(B组)、单纯牙周炎患者40例(C组),比较3组患者的空腹血糖(FPG)、空腹胰岛素(Fins)、三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6),3组患者的龈沟出血指数(SBI)、探诊深度(PD)。结果 A组和B组患者的血清TG、TC、LDL-C、FPG、Fins水平均显著高于C组(P <0. 05); A组和B组患者的血清HDL-C水平均显著低于C组(P <0. 05); A组和C组患者的血清TNF-α、IL-6水平均显著高于B组(P <0. 05);A组的血清TNF-α、IL-6水平显著高于C组患者(P <0. 05); A组和C组患者的SBI、PD水平均显著高于B组(P <0.05); A组的SBI、PD显著高于C组患者(P <0. 05);牙周炎伴T2DM患者随着FPG、Fins、TNF-α、IL-6水平升高,患者的牙周PD、SBI值越高,呈正相关(r值分别为0. 533、0. 574、0. 649、0. 628、0. 594、0. 614、0. 633、0. 671,P <0. 05)。结论牙周炎合并T2DM患者牙周疾病程度更加严重,牙周疾病的严重程度与血糖代谢紊乱、炎症反应加重具有一定的相关性。Objective To investigate the changes of serum glucose and lipid metabolism,serum inflammatory cytokines and their clinical significance in patients with periodontitis complicated by type 2 diabetes mellitus(T2DM).Methods Forty patients with periodontitis complicated by T2DM(group A),40 patients with simple T2DM(group B)and 40 patients with simple periodontitis(group C)were enrolled in the study.The fasting blood glucose(FBG),fasting insulin(Fins),triglyceride(TG),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),sulcus bleeding index(SBI)and probing depth(PD)were observed and compared between the two groups.Results The serum levels of TG,TC,LDL-C,FPG and Fins in group A and group B were significantly higher than those in group C(P<0.05).The serum levels of HDL-C in group A and group B were significantly lower than those in group C(P<0.05).The serum levels of TNF-αand IL-6 in group A and group C were significantly higher than those in group B(P<0.05).The serum levels of TNF-αand IL-6 in group A were significantly higher than those in group B(P<0.05).The levels of SBI and PD in group A and group C were significantly higher than those in group B(P<0.05).The levels of SBI and PD in group A were significantly higher than those in group C(P<0.05).With the increase of levels of FPG,Fins,TNF-αand IL-6,the PD and SBI values of patients with periodontitis complicated by T2DM were significantly increased,there was a positive correlation(P<0.05).Conclusion The severity of disease in patients with periodontitis complicated by T2DM is more severer,which may be caused by abnormal glucose metabolism and inflammatory response aggravation.
