摘 要:目的分析己糖激酶-2(HK-2)与肝癌的关系,评价该指标预测肝癌患者不良预后的临床价值。方法选取2013年1月至2014年12月普外科住院行肝癌根治性切除术的患者182例,采用Western blot检测肝癌组织中HK-2的表达情况。通过术后患者预后情况分为预后良好组和预后不良组。采用Kaplan-Meier方法计算并绘制患者生存曲线,并采用单多因素COX回归分析HK-2表达量及临床病理因素与肝癌根治术后患者预后的关系。建立ROC曲线分别评价潜在指标对肝癌根治术后患者预后的预测能力。结果肝癌患者术后2年内癌症复发转移及死亡的发生率为44. 5%。镜下脉管癌栓(P=0. 003)、肿瘤分期(P=0. 026)、血清AFU(P=0. 036)、HK-2蛋白表达(P=0. 001)是影响肝癌根治性切除术后2年肿瘤复发转移的独立影响因素(P <0. 05)。HK-2检测对预测患者预后的敏感性为62. 5%,特异性为83. 2%。结论预后不良组患者肝癌组织中HK-2蛋白表达量较高,HK-2表达有可能作为预测肝癌根治术后患者预后情况的一项检测指标。Objective To explore the relationship between the expression of hexokinase 2 (HK-2) protein and hepatocellular carcinoma (HCC), and to assess the clinical value of HK-2 in predicting the poor prognosis of patients with HCC. MethodsA total of 182 patients who underwent liver cancer curative resection in our hospital from January 2013 to December 2014 were enrolled in the study. The expression levels of HK-2 in HCC tissues were detected by Western Bblot. The patients were divided into good prognosis group and poor prognosis group after operation. Kaplan Meier method was used to calculate and draw the survival curve of patients, and single factor COX regression analysis was used to analyze the relationship between the expression of HK-2 and clinical pathological factors as well as and the prognosis of patients after hepatocellular carcinoma radical resection. ROC curve was established to evaluate the predictive ability of potential indexes for prognosis of patients after hepatocellular carcinoma radical resection. ResultsThe incidence rate of cancer recurrence, metastasis and death of patients with HCC within two years after operation was 44.5%. Endoscopic vascular tumor thrombus ( P <0.01), tumor staging ( P <0.05), serum AFU ( P <0.05) and HK-2 protein expression ( P <0.01) were the independent influencing factors on tumor recurrence and metastasis in 2 years after hepatocellular carcinoma radical resection ( P <0.05). The sensitivity and specificity of HK-2 detection for predicting the prognosis of patients were 62.5% and 83.2 %, respectively. ConclusionThe expression levels of HK-2 protein in poor prognosis group are higher than those in good prognosis group, and the expression levels of HK-2 protein may be regarded as a test index to predict the prognosis of patients after hepatocellular carcinoma radical resection.
