摘  要:目的探讨1. 5T磁共振弥散加权成像(DWI)对脑神经胶质瘤分级的诊断价值。方法选取2015年3月—2017年12月我院收治的经手术病理证实的脑神经胶质瘤60例临床资料,根据2007年WHO中枢神经系统肿瘤分类标准分为低分级组19例,高分级组41例。纳入者均行常规MRI平扫、增强扫描及DWI,测定肿瘤表观扩散系数(ADC)、r ADC值,比较不同病理分级脑神经胶质瘤ADC值、r ADC值,分析肿瘤ADC值与病理分级关系,并绘制ROC曲线分析ADC值对脑神经胶质瘤分级的诊断效能。结果低分级组ADC、r ADC值均高于高分级组(P <0. 05)。脑神经胶质瘤ADC值、r ADC值与病理分级呈负相关(r=-0. 722、-0. 731,P <0. 05)。ROC曲线分析显示,ADC值对脑神经胶质瘤分级诊断的最佳界值为1. 13×10^(-3)mm^2/s,敏感度为94. 36%,特异度为95. 72%。结论 DWI量化参数ADC在低分级、高分级脑神经胶质瘤中有着显著差异,对脑神经胶质瘤病理分级有较高价值。Objective To investigate value of 1.5T magnetic resonance(MR)diffusion weighted imaging(DWI)in diagnosis of classification of patients with brain glioma.Methods Clinical data of 60 brain glioma patients confirmed by operation and pathology during March 2015 and December 2017 was retrospectively analyzed,and the patients were divided into low-grade group(n=19)and high-grade group(n=41)according to WHO classification criteria for central nervous system tumors in 2007.All patients underwent conventional magnetic resonance imaging(MRI)plain scan,enhanced scan and DWI.The apparent diffusion coeffucient(ADC)and rADC values were determined,and relationship between tumor ADC value with pathological classification was analyzed.Diagnostic efficiency of ADC value for classification of brain glioma was analyzed by drawing receiver operating curve(ROC)curve.Results ADC and rADC values in low-grade group were significantly higher than those in high grade group(P<0.05).ADC and rADC values of brain glioma were negatively correlated with pathological classidication(r=-0.722,-0.731,P<0.05).ROC curve analysis showed that the best cut-off value,sensitivity and specificity of ADC value in classification diagnosis of brain glioma were 1.13×10^-3 mm^2/s,94.36%and 95.72%respectively.Conclusion There are significant differences of DWI quantized parameter ADC in low-grade and high-grade brain gliomas,and therefore it is of high value for pathological classification of brain glioma.
