手用ProTaper一次法根管治疗牙髓炎术后急症的临床分析

摘　　要：目的观察手用ProTaper一次法根管治疗牙髓炎术后急症临床疗效。方法方便选择该院2016年4月—2018年4月就诊的牙髓炎患者60例,根据治疗方法分为两组:手用ProTaper一次法根管治疗组(实验组,30例)和手用标准K锉一次法根管治疗组(对照组,30例),侧向加压技术充填根管,统计术后急症发生率以及影响因素。结果牙髓炎患者术后急症共发生17例,总发生率28.33%。其中实验组发生4例术后急症,发生率13.33%;对照组发生13例术后急症,发生率43.33%,两组差异有统计学意义(χ~2=14.76,P=0.01)。根据根管充填情况,超填组术后急症发生率显著高于另外两组(χ~2=36.18,P=0.01)。根据以上结果可认为影响术后急症发生率的因素为根管预备器械和根管充填情况。结论手用ProTaper一次法根管治疗方案并控制根管充填情况,能够有效降低牙髓炎患者术后急症的发生率,加快愈合速度,提高患者的生活质量。Objective To observe the clinical curative of hand proTaper one-visit root-canal therapy of emergency after the pulpitis surgery.Methods 60 cases of patients with pulpitis diagnosed in our hospital from April 2016 to April 2018 were convenient selected and divided into two groups according to different treatment methods,with 30 cases in each,the experimental group used the hand proTaper one-visit root-canal therapy,while the control group used the hand standardized K file one-visit root-canal therapy,and the incidence rate of emergency after surgery and its influencing factors were counted.Results There were 17 cases with postoperative emergency,and the total incidence rate was 28.33%,and in the experimental group,there were 4 cases with postoperative emergency,and the incidence rate was 13.33%,and in the control group,there were 13 cases with postoperative emergency,and the incidence rate was 43.33%,and the difference between groups was statistically significant(χ^2=14.76,P=0.01),and the incidence rate of postoperative emergency in the super filing group was obviously higher than that in the other two groups(χ^2=36.18,P=0.01),and the influencing factors of postoperative emergency included the root canal preparation instrument and filing condition.Conclusion The hand proTaper one-visit root-canal therapy and controlling the root canal filling condition can effectively reduce the incidence rate of postoperative emergency of patients with pulpitis,accelerate the healing rate and improve the quality of life of patients.
