胃癌患者60例血清及黏膜组织CK-18、CDH-17、CA724与CEA检测的临床价值

摘　　要：目的观察细胞角蛋白-18 (CK-18)、肝肠-钙黏连蛋白-17 (CDH-17)、糖类抗原724 (CA724)、癌胚抗原(CEA)在胃癌患者血清与胃黏膜组织中的表达水平,探讨联合检测对胃癌诊断的价值。方法选取经病理明确诊断为胃癌的患者60例(恶性病变组)、胃良性病变60例(良性病变组)和同期门诊健康体检者60例(正常对照组);采用酶联免疫吸附实验检测各组血清CK-18、CDH-17水平,电化学发光法检测血清CA724、CEA,同时采用免疫组化法检测CK-18、CDH-17、CA724、CEA在各组胃组织中的表达。结果 1)恶性病变组血清CK-18、CDH-17、CA724、CEA水平均较正常对照组、良性病变组明显升高(均P<0.05);2)胃癌组织中CK-18、CDH-17、CA724、CEA的阳性表达率均明显高于正常对照组、良性病变组(均P<0.05);3)Spearman分析显示:胃癌患者血清CK-18、CDH-17、CA724、CEA水平与胃癌组织中其表达水平呈正相关;4)联合检测CK-18、CDH-17、CA724、CEA在胃癌中的灵敏度和特异度分别为93.3%和81.7%,与单项检测胃癌灵敏度相比较,联合检测胃癌的灵敏度均升高(P<0.05)。结论检测血清CK-18、CDH-17、CA724、CEA的水平有助于胃癌的诊断,联合检测胃黏膜组织CK-18、CDH-17、CA724、CEA可提高胃癌诊断的灵敏度。Objective To investigate the expressions of cytokeratin-18(CK-18),cadherin-17(CDH-17),carbohydrate antigen-724(CA724)and carcinoembryonic antigen(CEA)in the serum and gastric mucosa tissues in the patients with gastric cancer,and analyse the diagnostic value of combined detection of CK-18,CDH-17,CA724 and CEA for gastric cancer.Methods A total of 60 patients with gastric cancer(malignant lesions group),60 patients with gastric benign lesion(benign lesion group)and 60 healthy people(control group)were selected for the study.The enzyme-linked immunosorbent assay(ELISA)was used to detect serum levels of CK-18 and CDH-17,and the electrochemical luminescence assay was used to detect serum levels of CA724 and CEA.Meanwhile,the immunohistochemical assay was used to detect the expressions of CK-18,CDH-17,CA724 and CEA in the gastric mucosa tissues.Results 1)Compared with the control group and the benign lesion,the levels of serum CK-18,CDH-17,CA724 and CEA in the malignant lesions group were significantly increased(all P<0.05).2)The positive rates of CK-18,CDH-17,CA724 and CEA in gastric cancer group were significantly higher than those in benign gastric group and control group(all P<0.05).3)Spearman correlation analysis showed that the levels of serum CK-18,CDH-17,CA724 and CEA were positively correlation with the expressions of CK-18,CDH-17,CA724 and CEA in the gastric cancer tissue.4)The sensitivity and specificity of combined detection of CK-18,CDH-17,CA724 and CEA in the diagnosis of gastric cancer were 93.3%and 81.7%,respectively.The sensitivity were significantly higher than those of detection of each of these markers alone(all P<0.05).Conclusion Detection of CK-18,CDH-17,CA724 and CEA in serum can be beneficial to diagnosis of gastric cancer,and combined detection of CK-18,CDH-17,CA724 and CEA in mucosal tissues can markedly improve the diagnostic sensitivity for gastric cancer.
