开窗减压术治疗含牙囊肿内埋伏牙的临床疗效观察

摘　　要：目的观察开窗减压治疗含牙囊肿内埋伏牙的效果。方法选取2015-2017年于合肥市口腔医院口腔颌面外科就诊的含牙囊肿患者16例,均采用开窗减压术配合术后使用囊肿塞,根据需要行间隙保持。术后随访24个月,观察囊肿恢复及埋伏牙萌出情况。结果术后随访发现,患者面部形态基本恢复正常,局部肿胀、膨隆消失,囊肿无复发。13例替牙期患者的20颗埋伏牙成功自行萌出,3例恒牙期患者的3颗埋伏牙经正畸牵引辅助后萌出,咬关系基本正常。影像学检查显示原颌骨低密度影消失,均见新生骨形成。结论开窗减压术治疗含牙囊肿内的埋伏牙效果确切。牙根未完全形成前行开窗减压治疗,囊肿内的埋伏牙常可自行萌出。Objective To observe the efficacy of windowing decompression for treating impacted tooth within dentigerous cyst. Methods Sixteen patients with impacted tooth within dentigerous cyst in the oral and maxillofacial surgery department of Hefei Municipal Dental Hospital during 2015-2017 were selected and adopted the windowing decompression combined with postoperative cyst plug,and conducted the space maintenance according to the demand.The postoperative follow up lasted for 24 months.The cyst recovery and impacted tooth eruption were observed. Results The postoperative follow-up revealed that the facial morphology basically recovered to the normal status.The local swelling and distension disappeared,the cyst had no relapse.Twenty impacted teeth in 13 cases of mixed dentition successfully spontaneously erupted.Three impacted teeth in 3 cases of permanent teeth erupted by orthodontic traction assist.The maxillary occlusal relation was basically normal.The imageological examination showed that the original jaw bone low density image disappeared and the new bone formation was found. Conclusion Windowing decompression has the definite effect for treating impacted tooth within dentigerous cyst.The windowing decompression is performed before the tooth root is not completely formed,the impact tooth within cyst often spontaneously erupts.
