胃腺癌根治术后标本与内镜活检标本中HER-2表达的一致性分析

摘　　要：目的:分析胃腺癌根治术后标本与内镜活检标本中人类表皮生长因子受体-2(HER-2)表达情况的一致性。方法:采用免疫组织化学(IHC)方法检测63例胃腺癌内镜活检标本的HER-2蛋白表达情况,用IHC及原位荧光杂交(FISH)方法检测相应胃腺癌切除术后标本的HER-2蛋白表达和基因扩增状况,将上述检测结果进行比较并结合临床病理特征进行分析。结果:63例胃腺癌根治术后标本中,共有8例HER-2过表达呈阳性,胃腺癌根治术后标本与内镜活检标本的IHC检测结果一致的比率为79. 36%,真阳性率和真阴性率分别为83. 3%和95. 9%。结论:胃腺癌内镜活检标本HER-2 IHC检测结果与根治术后标本HER-2检测结果存在高度的一致性,内镜活检标本HER-2蛋白IHC检测具有较好的预测价值。Objective:To research the consistent analysis of gastric adenocarcinoma HER-2 expression between radical operation specimens and endoscopic biopsy.Methods:From February 2014 to October 2015,63 patients from Jiuquan City People's Hospital were diagnosed with gastric adenocarcinoma using endoscopic biopsy specimens.The surgical specimens were gathered for pathological analysis.The relevant clinical and pathological data were collected.HER-2 protein expression of endoscopic biopsy specimens was detected by immunohistochemistry(IHC).HER-2 expression of protein and gene amplification condition of the corresponding tumor resection specimens were detected by IHC and fluorescence in situ hybridization(FISH).Results were analyzed for clinic pathological characteristics.Results:Among the 63 cases,8 cases were HER-2 positive,including 5 cases showing IHC(3+)and 3 cases showing IHC(2+)with FISH positive.The consistent rate among endoscopic biopsy and surgical operation specimens was 79.36%.The true positive rate and the true negative rate were 83.3%and 95.9%.Conclusion:There is highly consistency between the HER-2 detection of endoscopic biopsy specimen by IHC and the HER-2 detection of surgical specimens.HER-2 detection of endoscopic biopsy specimen by IHC shows favourable forecasting value.
