PET及PET/CT在检测头颈鳞状细胞癌残余及复发病灶价值的Meta分析

摘  要:目的系统评价PET及PET/CT对头颈鳞状细胞癌残余及复发病灶的诊断价值。方法计算机检索PubMed、The Cochrane Library、CNKI、WanFang Data、VIP、CBM和EMbase数据库,纳入PET或PET/CT诊断HNSCC放化疗后残留和复发病灶的研究,检索时限均从建库至2016年8月。由2位评价员按照纳入与排除标准独立筛选文献、提取资料和评价纳入研究的方法学质量,采用MeaDisc1.4软件进行Meta分析。结果最终纳入15篇前瞻性研究,共计795例患者。Meta分析结果显示对于原发灶处:SEN合并=0.81[95%CI(0.74,0.86)]、SPE合并=0.81[95%CI(0.78,0.86)]、+LR=4.55[95%CI(2.65,7.81)]、-L R=0. 2 5 [0. 1 5, 0. 4 3]、 DOR合并=23.42[95%CI(11.45,47.93)]、SROC曲线下面积(AUC)为0.90;对于颈部淋巴结处:SEN合并=0.74[95%CI(0.62,0.83)]、SPE合并=0.88[95%CI(0.84,0.91)]、+LR=6.05[95%CI(2.75,13.33)]、-LR=0.38[95%CI(0.23,0.64)]、DOR合并=23.40[95%CI(6.20,88.35)]、SROC曲线下面积(AUC)为0.87。结论 PET及PET/CT对头颈鳞状细胞癌残余及复发病灶具有较高的诊断价值,可用于HNSCC患者放化疗后残余及复发病灶的诊断。Objective To review the diagnostic value of PET or PET/CT for residual/recurrent head neck squamous cell carcinomas systematically.Methods We searched PubMed,The Cochrane Library,CNKI,WanFang Data,VIP,CBM and EMbase from inception to August,2016.Two reviewers screened literature according to the inclusion and exclusion criteria,extracted data,and assessed the quality of included studies.MetaDiSc 1.4 was used to conduct the meta-analysis.Results Fifteen studies were finally included.For residual/recurrent disease at the primary site,the pooled analysis shown:SEN=0.81[95%CI(0.74,0.86)],SPE=0.81[95%CI(0.78,0.86)],+LR=4.55[95%CI(2.65,7.81)],-LR=0.25[95%CI(0.15,0.43)],DOR=23.42[95%CI(11.45,47.93)]and SROC AUC=0.90.For residual/recurrent disease at the neck nodes,the pooled analysis shown:SEN=0.74[95%CI(0.62,0.83)],SPE=0.88[95%CI(0.84,0.91)],+LR=6.05[95%CI(2.75,13.33)],-LR=0.38[95%CI(0.23,0.64)],DOR=23.40[95%CI(6.20,88.35)]and SROC AUC=0.87.Conclusion PET and PET/CT are highly accurate in detecting residual/recurrent HNSCC.
