摘　　要：目的探讨α-平滑肌肌动蛋白(α-SMA)表达与进展期结直肠癌化疗疗效及预后的相关性。方法采用免疫组织化学方法,检测52例接受奥沙利铂联合氟尿嘧啶化疗的进展期结直肠癌患者标本中α-SMA的表达,分析其与临床病理因素和化疗疗效的相关性,并进行生存分析。结果 52例患者中,29例(55. 8%)为α-SMA高表达,α-SMA蛋白的表达与患者的年龄(X^2=0. 113,P=0. 730)、性别(X^2=0. 515,P=0. 332)、肿瘤部位(X^2=3. 675,P=0. 159)、组织分化程度(X^2=1. 852,P=0. 604)均无显著相关性。α-SMA高表达者化疗耐药率为65. 5%(19/29),显著高于α-SMA低表达者的13. 0%(3/23)(X^2=14. 470,P=0. 000)。α-SMA高表达者的无进展生存期显著短于α-SMA低表达者[(6. 4±1. 0)个月比(16. 0±3. 5)个月;χl2og-rank=5. 985,P=0. 018]。结论α-SMA高表达与进展期结直肠癌患者对奥沙利铂联合氟尿嘧啶类药物耐药相关。Objective To investigate the expression ofα-smooth muscle actin(α-SMA)in advanced colorectal cancer tissue and its association with chemotherapy response and prognosis.Methods The expression ofα-SMA was detected by immunohistochemistry in tissues from 52 advanced colorectal cancer patients who received oxaliplatin plus 5-fluorouracil regimen as first-line chemotherapy.Its relationship with clinical characteristics,chemotherapy response,and survival were analyzed.Results Of these 52 patients,29(55.8%)wereα-SMA overexpression,and the expression ofα-SMA protein was not significantly associated with the age(χ^2=0.113,P=0.730),gender(χ^2=0.515,P=0.332),tumor location(χ^2=3.675,P=0.159),and tissue differentiation(χ^2=1.852,P=0.604).The chemotherapy resistance rate was 65.5%(19/29)in patients with highα-SMA expression,which was significantly higher than that(13.0%,3/23)in patients with lowα-SMA expression(χ^2=14.470,P=0.000).Patients with highα-SMA expression exhibited a significantly shorter progression-free survival(PFS)compared with those with lowα-SMA expression[(6.4±1.0)months vs.(16.0±3.5)months;χ2log-rank=5.985,P=0.018].Conclusion Highα-SMA expression is associated with resistance to first-line chemotherapy and poor prognosis in advanced colorectal cancer patients.
