儿童急性髓细胞性白血病中B淋巴细胞瘤-2基因和P-糖蛋白表达的临床价值

摘　　要：目的探讨儿童急性髓细胞性白血病中B淋巴细胞瘤-2基因(Bcl-2)和P-糖蛋白(P-gp)的表达及临床价值。方法采用免疫组化S-P法检测初诊未治的100例儿童急性髓细胞性白血病和50例非恶性血液病患儿中Bcl-2和P-gp的表达,并分析两者的相关性。结果 100例儿童急性髓细胞性白血病骨髓细胞中Bcl-2和P-gp的阳性表达率分别为72.00%和44.00%,高于50例非恶性血液病患儿的4.00%和6.00%,差异均有统计学意义(χ~2=61.664、22.371,P均<0.001)。100例儿童急性髓细胞性白血病骨髓细胞中Bcl-2和P-gp的表达呈正相关关系(r_s=0.373,P<0.001)。结论儿童急性髓细胞性白血病的骨髓细胞中存在Bcl-2和P-gp蛋白的高表达,这可能有助于儿童急性髓细胞性白血病的诊断和预后判断。Objective To investigate the expressions and clinical value of B-cell lymphoma-2(Bcl-2)and P-glycoprotein(P-gp)in the children with acute myelogenous leukemia.Methods Immunohistochemical S-P method was used to detect the expressions of Bcl-2 and P-gp in the 100 cases of children with acute myelogenous leukemia and 50 cases of children with non-malignant hematological disease,and their correlation was analyzed.Results The positive expression rates of Bcl-2 and P-gp in the bone marrow cells of 100 cases of children with acute myelogenous leukemia were 72.00%and 44.00%,respectively,and were higher than those(4.00%and 6.00%)of the 50 cases of children with non-malignant hematological diseases,and the differences were statistically significant(χ^2=61.664,22.371;P all<0.001).There was a positive correlation between Bcl-2 and P-gp expressions in the bone marrow cells of 100 cases of children with acute myelogenous leukemia(r s=0.373,P<0.001).Conclusion Bcl-2 and P-gp protein are highly expressed in the bone marrow cells of children with acute myelogenous leukemia,which may be helpful for the diagnosis and prognosis of children with acute myeloid leukemia.
