摘 要:目的比较热牙胶充填系统2种根管充填方法的临床效果。方法选择2013年5月至2016年5月在安徽医科大学第二附属医院口腔科因牙髓病、根尖周病需行根管治疗的患者83例,共83颗患牙,按随机数字表法分为两组,试验组42例患者,42颗患牙(119个根管),对照组41例患者,41颗患牙(111个根管)。试验组患者采用热牙胶连续波加压充填+冷牙胶侧方加压法进行根管充填,对照组患者采用单纯热牙胶连续波加压充填法。比较2组患者充填方法在临床操作中容易出现的失误率(携热器带出牙胶尖、根管内气泡)、根充恰填率以及1年后临床成功率。结果试验组患者携热器向上拔出时牙胶尖带出根管数6根,对照组43根,差异有统计学意义(P<0.05)。根管内气泡实验组5根,对照组11根,差异无统计学意义(P>0.05)。试验组根管恰填113根,对照组根管恰填74根,差异有统计学意义(P<0.05)。根管治疗术后1年复查,试验组患者临床成功率为95.24%,对照组为85.37%,两组差异无统计学意义(P>0.05)。结论2种根管充填方法均能达到良好的临床效果,热牙胶连续波加压充填联合冷牙胶侧方加压法较单纯热牙胶连续波加压法充填根管操作失误少、根尖封闭效果更好。Objective To compare the clinical effects of two root canal filling methods in hot gutta-percha filling system. Methods A total of 83 patients with endodontic diseases or periapical diseases were randomly divided into two groups: 42 patients in experimental group with 42 teeth (119 root canals) and 41 patients in control group with 41 teeth (111 root canals). In experimental group, continuous wave vertical compaction technique combined with the cold lateral condensation technique were used, while in control group only continuous wave vertical compaction technique was used. Operational errors in treatment were recorded, like gutta-percha pulled out with the heat carrier, or cavities in filled canal, and the success rate of RCT after one year in two groups was compared. Results The gutta-percha points pulled out from root canal by the heat carriers happened 6 times in experimental group and 43 times in control group, with significant statistical difference ( P <0.05). The number of proper filled canals was 113 in experimental group and 74 in control group,which had statistical significance ( P <0.05). The success rate of RCT after one year was 95.24% and 85.37% respectively in experimental group and in control group, which had no statistical significance ( P >0.05). Conclusion The technique of continuous wave vertical compaction combined with cold lateral condensation has less errors and better apex closure, and both root filling techniques could achieve content clinical results.
