摘　　要：目的:探讨三维适形放疗联合顺铂同步化疗治疗宫颈癌的临床疗效,以及对血清上皮型钙黏蛋白(E-cad)、Fractalkine表达的影响。方法:选取2012年1月至2014年1月新疆维吾尔自治区人民医院收治的108例宫颈癌患者为研究对象,随机分为观察组和对照组,每组54例。对照组给予常规放疗+顺铂同步化疗,观察组给予三维适形放疗+顺铂同步放疗。比较两组肿瘤缓解情况、不良反应、生存率、血清E-cad、Fractalkine水平。结果:观察组治疗总有效率显著高于对照组(P<0.05)。观察组不良反应发生率低于对照组,3年生存率高于对照组,复发率和转移率低于对照组,差异均有统计学意义(均P<0.05)。治疗后,两组血清E-cad、Fractalkine水平显著上升,且观察组显著高于对照组(均P<0.01)。结论:三维适形放疗联合顺铂同步化疗治疗宫颈癌,可提高宫颈癌的临床疗效,上调E-cad和Fractalkine的表达,提高生存率,降低肿瘤转移率、复发率。Objective:To investigate the effects of three-dimensional conformal radiotherapy combined with cisplatin on the expression of serum E-cad and Fractalkine in patients with cervical cancer. Methods:From January 2012 to January 2014,a total of 108 patients with cervical cancer were selected,and randomly divided into an observation group and a control group,with 54 cases in each group.The patients in the control group received conventional radiotherapy combined with cisplatin,and those in the observation group were treated as the control group with addition of three-dimensional conformal radiotherapy.The tumor remission,adverse reaction,survival rate and serum E-cad and Fractalkine levels were compared. Results:The effective rate of the observation group was significantly higher than that of the control group( P <0.05).The incidence of adverse reactions,the recurrence rate and the transfer rate in observation group were lower than those in the control group,and 3-year survival rate was higher( P <0.05).The E-cad and Fractalkine levels of the observation group were higher than those in the control group( P <0.05). Conclusion:Three-dimensional conformal radiotherapy combined cisplatin could enhance the effectiveness of cervical cancer,elevate the E-cad and Fractalkine levels,increase the survival rate,and suppress tumor metastasis and recurrence.
