摘  要:目的研究常规X射线和X线断层融合(Digital Tomosynthesis,DTS)成像技术在下肢骨折固定术后骨痂形成过程中的诊断价值。方法选取2017年5月至2018年5月在我院进行固定手术的下肢骨折患者500例,在500例患者手术后1个月和6个月时采用常规X射线和DTS成像技术对所有患者进行检查,并对检测图像进行分析,对两位医师的图像分析结果一致性进行评价,统计X射线和DTS成像技术在术后1个月和术后6个月时的骨痂检出率、图像质量、辐射剂量以及两种技术的诊断效能。结果两位医师的图像分析结果一致性较高。DTS成像技术在术后1个月时和术后6个月时的骨痂检出率显著高于常规X射线技术检测下的骨痂检出率,具有统计学差异(P<0.05);DTS成像技术在术后1个月和6个月时的图片质量2分分布率显著高于X射线技术图片质量的2分分布率,图片质量0分分布率显著低于X射线技术图片质量的0分分布率,差异具有统计学意义(P<0.05);DTS成像技术的辐射剂量相关参数均明显低于常规X射线,差异具有统计学意义(P<0.05);DTS成像技术的特异度、敏感度和准确度均显著高于X射线技术,差异有统计学意义(P<0.05)。结论 DTS成像技术在下肢骨折固定术后的愈合诊断过程中,能够准确地对患者的骨痂情况进行检查,且患者所受辐射剂量较低,对患者的恢复及预后诊断提供了重要的帮助价值。Objective To study the diagnostic value of conventional X ray and Digital tomosynthesis (DTS) imaging in the process of callus formation after lower limb fracture fixation. Methods A total of 500 cases of lower limb fracture patients who underwent fixation surgery in our hospital from May 2017 to May 2018 were examined by conventional X ray and X ray fault fusion imaging at 1 and 6 months after operation, and the images were analyzed. The consistency of the image analysis results of two doctors was evaluated. The detection rate of callus, image quality, radiation dose and diagnostic efficiency of two techniques were measured by X-ray and X-ray tomography fusion imaging technique at 1 months after operation and 6 months after operation. Results The image analysis results of two doctors were consistent. The detection rate of callus in 1 months after operation and 6 months after operation was significantly higher than that of conventional X ray technique, with statistical difference (P<0.05). The 2 distribution rate of image quality was significantly higher than that of X ray technique in 1 months and 6 months after the operation, and the distribution rate of the image quality was significantly higher than that of the X ray technique. The rate of DTS imaging was significantly higher than that of the X ray technique. The 0 sub distribution rate of the image quality was significantly lower than that of the X ray technique. The difference was statistically significant (P<0.05). The radiation dose related parameters of the DTS imaging technique were significantly lower than that of the conventional X ray (P<0.05), and the specificity, sensitivity and accuracy of the DTS imaging technique were all obvious.
