躯干核心肌群强化训练的强度对老年脑卒中患者平衡能力的疗效研究

摘　　要：目的探讨躯干核心肌群训练的时间强度对老年脑卒中患者平衡能力的疗效。方法采用病例对照的研究方法,选取2013年4月～2013年10月在华中科技大学同济医学院附属协和医院康复医学科住院治疗的45例老年脑卒中所致偏瘫患者,按照随机数表法随机分为A、B、C 3组,每组15例,均给予常规治疗,并在此基础上联合躯干核心肌群训练,A组1次/d,B组2次/d,C组3次/d,每次30 min,每周5 d,共训练3个月。在治疗前及治疗后1、2和3个月时分别采用Berg平衡量表对3组患者平衡功能进行评定。结果治疗前3组患者平衡能力评分比较,差异无统计学意义(P> 0.05);治疗1、2和3个月后,3组患者的平衡量表评分都明显提高,组内比较差异均有统计学意义(P <0.05);1和2个月后组间比较,平衡能力评分C组高于B组,B组高于A组,差异均有统计学意义(P <0.05),但3个月后,3组患者平衡能力评分,组间比较差异均无统计学意义(P>0.05)。结论增加治疗剂量,提高躯干核心肌群强化训练的时间强度可以加快老年脑卒中患者早期平衡功能的恢复,但随着治疗时间延长,远期疗效并无差异。Objective To explore Effect of Intensive Training of Trunk Core Muscles on Balance Ability of Elderly Stroke Patients.Methods This study used case-control methods.45 patients with hemiplegia were involved.According to random digital table method,15 cases were randomly divided into A,B and C,The three groups were given regular treatment,On this basis,the core muscle training is combined with the core body,once/day in group A,twice/day in group B,thrice/day in group C,and thirty minutes at a time,five days a week,and total training time was three months.The Berg balance scale(BBS)was used to assess the balance function before treatment and 1 month,2 months and 3 months after treatment.Results The balance ability were all improved obviously after training.And there were statistically significant difference among three groups at 1 month and 2 months after treatment,(P<0.05).But There was no statistically significant difference among the three groups at 3 months after treatment(P>0.05).Conclusions The core muscle group strengthening training can accelerate the recovery of early balance function in the elderly stroke patients,However,the long-term effect is not obvious.
