摘 要:目的探讨高度近视人群脉络膜厚度的分布情况。方法资料来自北京眼病研究2011,以人群为基础的横断面研究方法。等效球镜度≤-6. 00 D并满足测量要求的78例(103只眼)纳入高度近视组,对照组随机抽取年龄及性别匹配的227例(227只眼)正常眼。使用谱域相干光断层扫描术(EDI SD-OCT)对黄斑区进行扫描,专用软件测量脉络膜厚度。结果两组均为中心凹下脉络膜厚度最大,两组差异有统计学意义。对照组(263. 0±92. 9)μm高度近视组(110. 6±85. 2)μm,对照组最小值出现在中心凹鼻侧略偏下方,平均厚度为(182. 2±89. 7)μm,高度近视组最小值出现在鼻下方,平均(60. 0±62. 7)μm。结论在高度近视人群中,脉络膜平均厚度明显低于正常人群,脉络膜的血管状态及血流灌注变化可能对高度近视的眼底改变有密切的影响。Objective To investigate the distribution of subfoveal choroidal thickness in highly myopic eyes.Methods Our study was based on the Beijing Eye Study 2011, a population-based, cross-sectional study in northern China. 103 high myopic eyes (≥-6.00 diopters) and 227 normal eyes which randomly selected from the baseline, with age and gender matched in proportion to the population. A detailed ophthalmic examination was performed including spectral-domain optical coherence tomography (SD-OCT) with enhanced depth imaging for measurement of subfoveal choroidal thickness. The measurements were performed using the Heidelberg Eye Explorer software.Results The subfoveal choroidal thickness was the thickest in both groups. The mean subfoveal choroidal thickness was (110.6±85.2)μm in high myopic group and (263.0 ± 92.9)μm in control group ( P <0.001). The mean regional choroidal thickness was minimum of (182.2±89.7)μm at nasal and slightly inferior to the fovea in control group, and was the smallest of (60.0± 62.7)μm at nasal and inferior macula in high myopic group.Conclusions The subfoveal choroidal thickness in high myopic eyes was significantly thinner than that of normal eyes. Abnormalities of the choroid blood perfusion may play a role in the pathogenesis of myopic degeneration.
