摘  要:目的:通过使用锥束计算机断层扫描技术(Cone-beamcomputed tomography,CBCT),对成人骨性Ⅱ类患者上气道影像进行测量,分析骨性Ⅱ类患者与阻塞性睡眠呼吸暂停低通气综合征(Obstructive sleep apnea-hypopnea syndrome,OSAHS)之间的相关性,为临床诊断及治疗OSAHS提供理论依据。方法:选取2015-2018年在广西医科大学附属口腔医院口腔正畸科就诊的38例成人骨性Ⅱ类(ANB角>5°)患者,按下颌角高度分为均角组(22°<Go-Gn-SN<32°)及高角组(Go-Gn-SN≥32°),拍摄颅颌面部CBCT,并测量上气道的横截面积、高度、长度、矢状径,运用MIMICS15.0软件测量上气道容积指标,比较均、高角组两组间差异并进行组间的相关性分析。结果:均角组与高角组上气道各段的线距测量结果差异均有统计学意义(P<0.05),均角组的腭咽、舌咽及鼻咽段高度显著大于高角组。结论:患者的垂直骨面型可影响与其上气道的形态结构。Objective This study used CBCT to measure the upper airway image of adult patients with skeletal class Ⅱ. The correlation between skeletal classⅡpatients and OSAHS was analyzed, which provided a theoretical basis for clinical diagnosis and treatment of OSAHS. Methods The adult skeletal class Ⅱ(ANB angle>5°) patients who were admitted to the department of Orthodontics, Affiliated Stomatological Hospital of Guangxi Medical University from 2015 to 2018 were selected and divided into the average angle group (22°<Go-Gn-SN<32°) and high-angle group (Go-Gn-SN≥32°), a total of 38 cases. The craniofacial CBCT was taken.The CBCT technique was used to measure the upper airway cross-sectional area, height,length, and sagittal diameter. The airway volume was measured with MIMICS 15.0 software, and the differences between the high and average angle groups were compared,the correlation analysis between the two groups was performed. Results There were significant differences in line spacing measurements between the average angle group and the high angle group (P <0.05).The height of the pharyngeal, pharyngeal and nasopharyngeal segments of the average angle group was significantly greater than that of the high angle group. Conclusion The morphological structure of the upper airway can be affect by the patient's vertical bone pattern.
