摘  要:目的探讨局部皮瓣在修复头面部皮肤恶性肿瘤软组织缺损的可行性。方法选择2015年6月至2018年10月辽宁省肿瘤医院收治的头面部皮肤恶性肿瘤13例为研究对象,根据缺损部位、面积和形状选择皮瓣类型。6例采用叶状皮瓣,3例采用菱形皮瓣,4例采用V-Y推进皮瓣于广泛切除术后行局部皮瓣移植修复皮肤缺损。结果 1例出现皮瓣部分坏死,经局部换药处理后延迟愈合,其余病例均一期愈合,经6~28个月(平均17个月)随访,皮瓣色泽、质地均佳。尚无复发及死亡病例。结论局部皮瓣在头面部皮肤恶性肿瘤修复重建的方式效果确切,可达到最佳修复效果,值得临床推广和应用。Objective To investigate the feasibility of local flap in repairing soft tissue defects of head and face skin malignant tumors. Methods Thirteen cases of head and face skin malignant tumors were treated in Liaoning Cancer Hospital from June 2015 to October 2018. The types of skin flaps were selected according to the location, area and shape of the defect. Leaf flaps were used in 6 cases, rhomboid flaps in 3 cases, and V-Y push flaps were used in 4 cases to repair skin defects after extensive resection. Results One case of skin flap necrosis occurred, and delayed healing after local dressing treatment. The other cases were healed in the first stage. After 6 to 28 months, the average follow-up was 17 months. The color and texture of the flap were good. There were no deaths or recurrences. Conclusions The local flap is effective in the repair and reconstruction of malignant tumors of the head and face skin. It can achieve the best repair effect and is worthy of clinical promotion and application.
