摘 要:目的探讨3D-ASL技术在无强化小脑胶质瘤与小脑梗死鉴别诊断中的应用价值。方法回顾性分析经临床病理或随诊证实的23例无强化小脑病变患者的3D-ASL检查结果,其中胶质瘤9例,小脑梗死14例,所有患者均行3D-ASL检查,测量病灶区及镜像对照区CBF值。结果 23例小脑病变均呈长T_1长T_2信号,增强扫描均无强化;3D-ASL检查显示14例小脑梗死呈低灌注;9例胶质瘤呈等高灌注;胶质瘤病变区灌注高于小脑梗死病灶区。结论 3D-ASL在无强化小脑胶质瘤与小脑梗死的鉴别诊断中有重要的应用价值。Objective To evaluate the value of 3D whole-brain pseudo-continuous arterial spin labeling imaging in diagnosis of non-enhanced cerebellar gliomas and cerebellar infarction. Methods 3D-ASL examination results were retrospectively analyzed in 23 patients with clinically diagnosed cerebellar lesions, including 9 cases of gliomas and 14 cases of infarcion. All patients were performed 3D-ASL scanning with measurement of CBF values. Results All 23 patients showed long T 1 long T 2 and non-enhanced abnormal signal. 14 cases with cerebellar infarction showed hypo-perfusion in 3D-ASL. 9 cases with glioma showed hyper-perfusion.The CBF values were higher in the gliomas than in the infarction. Conclusion 3D-ASL imaging technique is of great value in differential diagnosis of non-enhanced cerebellar gliomas and cerebellar infarction.
