微创手术治疗妊娠合并卵巢良性肿瘤的效果及安全性观察

摘　　要：目的对于妊娠合并卵巢良性肿瘤的治疗中微创手术的效果及安全性进行观察,以期为进一步提高妊娠合并卵巢良性肿瘤手术治疗的合理选择提供参考。方法对郑州大学第五附属医院于2015年8月—2017年6月期间接收的64例经临床确诊为妊娠合并卵巢良性肿瘤患者的临床资料进行回顾性分析,并将患者资料按照治疗方式的不同分为对照组(30例)与观察组(34例),对照组采用传统开腹手术,观察组采用微创手术—腹腔镜手术治疗,对两种手术方式的临床效果及安全性进行对比观察。结果与对照组相比,在手术成功率、分娩情况、胎儿Apgar评分上,观察组数据差异无统计学意义(P>0.05);在手术时间、术中出血量、排气时间、住院时间及术后并发症上,两组差异有统计学意义(P<0.05)。结论在妊娠合并良性卵巢肿瘤的治疗中运用微创手术对产妇卵巢肿瘤的治疗并顺利完成妊娠具有积极作用,对患者造成术后损伤较传统开腹手术少,可加快患者术后恢复且具有较高安全性,对患者预后有利,值得推广。Objective To observe the effect and safety of minimally invasive surgery in the treatment of benign ovarian neoplasms in pregnancy,so as to provide reference for further improving the choice of surgical treatment for benign ovarian neoplasms. Methods The clinical data of 64 patients with benign ovarian tumor confirmed by clinical diagnosis from August, 2015 to June, 2017 were retrospectively analyzed. The patients were divided into control group(30 cases)and observation group(34 cases)according to different treatment methods. The control group were treated with traditional laparotomy,and the observation group were treated with minimally invasive surgery and laparoscopic surgery. The clinical effect and safety of different surgical methods were compared and observed. Results Compared with the control group,there was no significant difference in the operative success rate,delivery situation and fetal Apgar score in the observation group(P>0.05), but in the operation time, blood loss during operation, exhaust time, hospital stay and postoperative complications,the observation group was less than the control group(P<0.05). Conclusion Minimally invasive surgery is effective in the treatment of pregnancy with benign ovarian neoplasms,has a positive effect on the successful completion of pregnancy,and causes less postoperative injury to the patients than the traditional open surgery. It can accelerate postoperative recovery and has higher safety. It is beneficial to the prognosis of patients,and is worth popularizing.
