颌骨骨化纤维瘤28例临床及病理学分析

摘　　要：目的:对28例骨化纤维瘤(OF)进行系统的临床病理分析。方法:收集蚌埠医学院第一附属医院1998—2015间诊断为OF的临床、影像及病理学资料,进行三型分类,分析性别、年龄、影像、血清、治疗方法和随访资料,并进行统计学分析。结果:青少年砂瘤型骨化纤维瘤(JPOF)好发于女性,上颌骨2例,生长均累及鼻窦;多发患者1例;血清碱性磷酸酶升高患者4例;8例患者(28.6%)出现牙齿移位,3例(10.7%)出现截根状牙根吸收。随访16例,复发7例,均为首次保守治疗的患者。结论:OF分为传统型骨化纤维瘤(COF)、青少年砂瘤型骨化纤维瘤(JPOF)和小梁型骨纤维瘤(JTOF);青少年砂瘤型发病性别、部位有特点;患者牙齿移位易见,部分患者牙齿还会发生截根状吸收;年轻患者更易出现碱性磷酸酶升高;复发与手术方式有关。Objective: To analyze the clinical and pathological data of ossifying fibroma (OF) of the jaws. Methods: 28 patients diagnosed as OF, were retrieved from 1998 to 2015. The clinical data, age, imaging performance, serology, pathologic features and their prognosis were retrospectively reviewed. The statistical analysis was performed. Results: The conventional OF was diagnosed in 22 patients;juvenile prammomatoid OF(JPOF) was diagnosed in 4 patients, and juvenile trabecular OF (JTOF) in 2 patients. JPOF had the female prevalent, 2 occurred in the maxilla, 2 involved the paranasal sinuses. Mutiple OF was found in one case. 4 cases revealed elevated alkaline phosphatase levels. 8 patients had tooth displacement, and root resorption occurred in 3 patients. The follow-up data were acquired in 16 patients. 7 patients had recurrence which underwent the primary treatment with enucleation. Conclusion: COF was the most common type, while JPOF and JTOF were relatively rare. JPOF has characteristics on age, gender and location. Tooth displacement was often seen, some root appeared as an amputated-like resorption. Younger patients were more likely to have elevated ALP. Treatment method had the relationship with the recurrence.
