摘 要:目的:分析窄带成像放大胃镜联合靛胭脂染色在早期胃癌及癌前病变诊断中的应用价值。方法:选取惠州市第一人民医院消化内科2015年1月至2017年12月收治1 250例胃癌高危人群,均予以传统白光内镜检查和窄带成像(NBI)放大胃镜联合靛胭脂染色检查,以活检病理检查诊断结果作为金标准,同时选择同期1 250例非胃癌高危人群进行比较分析。结果:NBI放大胃镜联合靛胭脂染色的诊断准确率为94.74%,高于传统白光内镜73.68%(P<0.05);NBI放大胃镜联合靛胭脂染色在胃癌高危人群和非胃癌高危人群中胃癌检出率分别为2.88%和0.56%,均高于传统白光内镜胃癌检出率,差异具有统计学意义(P<0.05)。结论:NBI放大胃镜联合靛胭脂染色检查在早期胃癌及癌前病变诊断中的应用价值较高。Objective:To analyze the application value of narrow band imaging(NBI)magnifying gastroscope combined with indigo carmine staining in the diagnosis of early gastric cancer and precancerous lesions.Methods:A total of 1,250 high-risk population of gastric cancer admitted to department of gastroenterology of the first people's hospital of Huizhou from January 2015 to December 2017 were selected.All of them were examined by traditional white light endoscopy and NBI magnifying gastroscopy combined with indigo carmine staining.The diagnostic results of biopsy and pathological examination were taken as the gold standard,and 1,250 non-gastric cancer high-risk population in the same period were selected for comparative analysis.Results:The diagnostic accuracy of NBI magnifying gastroscope combined with indigo carmine staining was 94.74%,which was higher than that of traditional white light endoscopy(73.68%)(P<0.05).The detection rate of NBI magnifying gastroscopy combined with indigo carmine staining was 2.88%and 0.56%,respectively,in high-risk population of gastric cancer and non-high-risk population of gastric cancer,both significantly higher than the detection rate of conventional white light endoscopy for gastric cancer,and the difference was statistically significant(P<0.05).Conclusion:NBI magnifying gastroscopy combined with indigo carmine staining is of high application value in the diagnosis of early gastric cancer and precancerous lesions.
