窄带成像技术联合放大内镜对Barrett食管的诊断价值

摘　　要：目的:探讨窄带成像技术联合放大内镜诊断Barrett食管(Barrett Esophagus,BE)的效果。方法:选择2015年10月至2017年3月间30例经胃镜检查确诊为Barrett食管的患者进行研究,患者先接受普通内镜联合放大内镜检查,视作普通组,之后接受放大内镜联合内镜窄带成像技术(NBI)检查,视作NBI组,比较检查结果。结果:NBI组黏膜腺管开口形态、BE病变轮廓、浅表毛细血管结构形态的图像清晰度评分均高于普通组(P<0.05);NBI组腺管开口分型与病理结果符合率为93.33%,NBI组对于肠上皮化生的检出率为87.50%,与病理结果的差异无统计学意义(P>0.05)。结论:窄带成像技术联合放大内镜用于诊断Barrett食管准确率高,能够实现Barrett食管的早期诊断,可在临床推广。Objective: To probe into the effects of narrow-band imaging technique combined with magnifying endoscopy for Barrett esophagus. Methods: 30 cases of patients with Barrett esophagus and who were diagnosed by gastroscopy examination from October,2015 to March,2017 were selected for the research, the patients were checked through the general endoscopy combined with magnifying endoscopy as the general group,and then were checked through the magnifying endoscopy combined with endoscopic narrow-band imaging (NBI) as the NBI group.Then,the results of the examination were compared. Results: The score of the image clarity of mucosal duct opening morphology, BE lesion profile and superficial capillary structure in the NBI group was higher than those in the normal group ( P <0.05).The coincidence rate between the opening classification of the adenoducts and the pathological results was 93.33 %, and the detection rate of intestinal metaplasia in group NBI was 87.50 %. Conclusion: The accuracy of narrow-band imaging technique combined with magnifying endoscopy in diagnosis of Barrett esophagus is higher,which can achieve the early diagnosis of Barrett esophagus,thus it can be promoted in the clinic.
