摘 要:目的评估在正畸治疗中存在牙槽骨骨量不足的患者应用自体牙骨粉进行牙槽骨增量的临床效果。方法选择牙周状态稳定、可以进行手术的正畸固定矫治患者4例(植骨位点17个),锥形束CT (CBCT)显示牙齿唇侧骨皮质菲薄缺失,矫治计划需要拔除其他牙齿且拔除数量能够制成足够的自体牙骨粉。通过CBCT测量术前、术后3个月、术后6个月同一植入位点不同高度处牙槽骨厚度的变化情况。结果术前植入位点牙槽嵴顶、根方3 mm、根方6 mm处牙槽骨厚度分别为(7.91±3.58)、(9.16±3.23)、(10.10±3.36) mm,术后3个月平均厚度分别为(10.10±3.31)、(11.90±3.36)、(12.90±4.27) mm,术后6个月平均厚度分别为(8.86±3.27)、(10.50±3.49)、(11.60±4.10) mm。术后3个月、术后6个月与术前相比,牙槽骨厚度均显著增加(P <0.05),术后6个月与术后3个月相比,牙槽骨厚度减小(P <0.05)。结论将自体牙骨粉应用于牙槽骨菲薄缺失患者的正畸治疗中,可以扩大牙齿移动安全范围,虽然术后牙槽骨有吸收,但厚度较术前仍明显改善。Objective To evaluate the clinical effect of alveolar bone augmentation by applying autogenous tooth bone graft material to patients with alveolar bone deficiency in orthodontic treatment.Methods Four patients with a stable periodontal condition managed with fixed orthodontic treatment were included,and the number of graft sites was 17.Cone beam computed tomography(CBCT)showed labial alveolar bone deficiency.The treatment plan included extraction,and the extracted teeth were used to prepare autogenous tooth bone material.The alveolar bone width was measured at the same site at multiple heights prior to operation and at 3 and 6 months post-operation.Results The alveolar bone width was higher at both 3 and 6 months post-operatively(P<0.05)than that pre-operatively.The alveolar bone width at 6 months post-operation was lower than that at 3 months post-operation(P<0.05).Conclusion Applying autogenous tooth bone graft material to patients with alveolar bone deficiency in orthodontic treatment can expand the range of tooth movement and result in good clinical outcomes.
