摘  要:目的探讨血清胱抑素C(Cys-C)水平在脑梗死后血管性认知功能障碍的相关性。方法选择脑梗死后认知功能障碍组患者82例,脑梗死后认知功能正常对照组41例,检测血清Cys-C水平,认知功能评分,调整可能的影响因素,分析血清Cys-C水平与认知功能障碍发生的关系。结果不同Cys-C水平组中年龄、糖尿病、高血压、吸烟、饮酒、血脂异常、肌酐,结果异常情况分布差异有统计学意义(P<0.05)。认知功能障碍组血清Cys-C水平(1.35±0.47)mg/L与对照组(1.07±0.40)mg/L差异有统计学意义(t=20.56,P<0.01)。认知功能障碍组血清Cys-C水平与MOCA得分(r=-0.562)均存在负相关(P<0.05),随着血清Cys-C水平的升高,认知功能障碍患病的发生率升高(χ2=14.15,P<0.01)。不同血清Cys-C水平患者,认知功能障碍患病风险的多因素Logistic回归分析显示,高血清Cys-C水平是导致认知功能障碍的独立危险因素。结论血清高Cys-C水平与脑梗死后认知功能损害发生风险密切相关。Objective To investigate the relation between serum cystatin C (Cys-C) level and vascular cognitive impairment after cerebral infarction. Methods 82 patients with cognitive dysfunction after cerebral infarction and 41 cases of normal cognitive function after cerebral infarction were selected. The level of serum Cys-C,cognitive function score,the possible influencing factors were adjusted,and the relationship between the level of serum Cys-C and the occurrence of cognitive dysfunction was analyzed. Results There were significant differences in the distribution of age,diabetes,hypertension,smoking,drinking,dyslipidemia and creatinine in different Cys-C groups (P<0.05). The level of serum Cys-C in cognitive impairment group (1.35±0.47)mg/L was significantly different from that in control group (1.07±0.40)mg/L(t=20.56,P<0.01). There was a negative correlation between the serum Cys-C level and the MOCA score (r=-0.562) in the cognitive impairment group (P<0.05),and the incidence of cognitive impairment increased with the increase of serum Cys-C level(t=14.15,P<0.01). Multiple factor Logistic regression analysis of the risk of cognitive impairment in patients with different serum Cys-C levels showed that high serum level of Cys-C was an independent risk factor for cognitive dysfunction. Conclusion The high serum Cys-C level is closely correlated with the occurrence risk of mild cognitive impairment after cerebral infarction.
