CA19-9、PGⅠ及PGⅡ在胃癌患者血清和组织中表达及其临床意义

摘　　要：目的分析糖类抗原19-9(CA19-9)、胃蛋白酶原Ⅰ(PGⅠ)、胃蛋白酶原Ⅱ(PGⅡ)在胃癌患者血清和组织中表达及其临床意义。方法选取2016年6月至2018年2月我院接受胃镜检查的患者110例为研究对象,其中经胃镜及病理组织学检查确诊为胃癌者60例(记为胃癌组),胃良性病变者50例(记为良性组),另选择同期入院体检的健康者30例为对照组,检测三组血清CA19-9、PGⅠ及PGⅡ水平,并比较胃癌患者中胃癌组织、癌旁组织、正常组织CA19-9、PGⅠ及PGⅡ水平,分析CA19-9、PGⅠ及PGⅡ对胃癌的诊断效能。结果胃癌组血清CA19-9高于良性组、对照组,而PGⅠ及PGⅡ水平低于良性组、对照组,良性组、对照组上述指标对比组间的差异也有统计学意义(P <0. 05);胃癌患者中,癌组织CA19-9阳性率高于癌旁组织、正常组织,而PGⅠ及PGⅡ阳性率低于癌旁组织、正常组织,对比组间的的差异具有统计学意义(P<0.05);ROC曲线分析结果显示,CA19-9、PGⅠ、PGⅡ联合检测诊断胃癌的灵敏度、特异性、曲线下面积分别为0.86、0.91、0.847,均高于三项指标单独诊断。结论 CA19-9在胃癌患者血清和癌组织中均呈高表达,而PGⅠ及PGⅡ呈低表达,三者联合检测对胃癌有较高诊断价值,值得在临床推广实践。Objective To analyze the expressions of carbohydrate antigen 19-9(CA19-9),pepsinogen I(PGI)and pepsinogen II(PGII)in serum and tissues of patients with gastric cancer and their clinical significances.Methods A total of 110 patients who underwent gastroscopy in the hospital from June 2016 to February 2018 were selected as the study subjects.Among them,there were 60 patients with gastric cancer(the gastric cancer group)and 50 patients with benign gastric lesions(the benign group)diagnosed by gastroscopy and histopathological examination.30 healthy subjects who underwent physical examination in the hospital during the same period were also selected as the control group.Serum CA19-9,PGI and PGII levels in all groups were detected,and the levels of these markers in gastric cancer tissues,adjacent tissues and normal tissues of patients with gastric cancer were also compared.The diagnostic efficiencies of CA19-9,PGI and PGII for gastric cancer were then analyzed.Results The serum CA19-9 level in the gastric cancer group was higher than that in the benign group and the control group,while the PGI and PGII levels were lower than those in the benign group and the control group,(P<0.05).The positive rate of CA19-9 in cancer tissues was higher than that in adjacent tissues and normal tissues,while the positive rates of PGI and PGII were lower than those in adjacent tissues and normal tissues,(P<0.05).ROC curve analysis showed that the sensitivity,specificity and the area under the curve of the combined detection of CA19-9,PGI and PGII were 0.86,0.91 and 0.847,respectively,which were higher than those of the three individual tests.Conclusion The expression of CA19-9 is relatively high in the serum and cancer tissues of patients with gastric cancer,while the expressions of PGI and PGII are relatively low.The combined detection of the three markers is of high diagnostic value for gastric cancer.
