时间医学在鼻咽癌化疗中的运用

摘　　要：近年来许多临床研究证明肿瘤组织与生物机体正常的组织存在明显不同的生物节律,并且在基因、细胞周期、药物代谢动力学、端粒酶、肿瘤血供、肿瘤体温变化、内生保护剂方面均存在显著生物节律差异。大量研究成果发现卡铂、5-FU、生物反应调节剂等疗效在不同时间上具有较大差异。同时,治疗鼻咽癌的常见化疗药有5-FU、铂类以及多西他赛等,为时间医学在鼻咽癌的时辰治疗可行性方面提供了有力的证明。并且,时间治疗在提高鼻咽癌患者的生活质量,减少毒副反应,加大化疗药物剂量方面有很大的进展。然而,时间治疗结果也存在巨大变异性,影响包括性别、年龄、体力活动、疾病状态、遗传多态性和个体表型特性,本研究对上述内容进行了系统的回顾和探讨,以期为后续临床研究提供参考。In recent years,many clinical studies had shown that the biological rhythm of tumor tissues and organisms normal tissue was different significantly,so as to the genes,cell cycle,pharmacokinetic,telomerase, tumor blood supply,temperature changes,there exist significant biological rhythm differences endogenous protective agent.A large number of studies had have found that the curative effects of carboplatin,5-FU,and biological response modifier had respectively differences at different times.At the same time, common chemotherapy drugs for nasopharyngeal cancer include 5-FU,platinum and docetaxel,which provide a strong evidence for the feasibility of chronomedicne in the treatment of nasopharyngeal cancer.Moreover,time therapy has made great progress in improving the quality of life of patients with nasopharyngeal carcinoma,reducing adverse reactions and increasing the dose of chemotherapy drugs.However,the chronomedicne results also have great variability,including gender,age,physical activity,disease status,genetic polymorphism and individual phenotypic characteristics.This study conducted a systematic review and discussion of the above contents in order to provide reference for subsequent clinical studies.
