摘 要:目的探讨前列腺癌图像引导放疗(IGRT)中KV级正交X线和锥形束计算机断层扫描(CBCT)两种引导方法对前列腺癌摆位误差的差异。方法选取25例内植标志物的中危前列腺癌患者作为研究对象。全部患者于放疗前先后接受KV级正交X线和基于软组织灰度值校准的CBCT两种图像引导方法。以5 mm、3 mm分别作为误差允许范围,记录并比较KV级正交X线和基于软组织灰度值校准的CBCT在左右(LR)、头脚(SI)和前后(AP)3个方向上的位移情况。结果 KV级正交X线方法在LR、SI、AP方向上的位移为(0.20±0.19)mm、(1.44±1.09)mm、(1.51±1.26)mm;CBCT方法在LR、SI、AP方向上的位移为(1.27±1.18)mm、(4.08±3.75)mm、(4.25±3.82)mm。两种方法在3个方向上位移误差≤5 mm的分次百分比均在70%以上。结论 KV级正交X线和CBCT两种图像引导方法对前列腺癌放疗中的摆位误差差异较小,患者需根据临床具体的实际情况选择自适应的方法。Objective To explore the difference of setup error between KV-grade orthogonal X-ray photography and cone beam computerized tomography(CBCT)in image-guided radiation therapy(IGRT)for patients with prostate cancer.Method Twenty-five moderate-risk prostate cancer patients with implanted markers were selected as objects,who received both of the KV-grade orthogonal X-ray photography photography and CBCT scan based on soft tissue gray value before radiotherapy.Taking 5 mm and 3 mm as the allowable scope of error,the average value and standard deviation of displacement of the two methods on left-right(LR),superior-inferior(SI)and anterior-posterior(AP)directions were recorded and compared.Result The average value and standard deviation of displacement on LR,SI and AP of the KVgrade orthogonal X-ray photography was(0.20±0.19)mm,(1.44±1.09)mm,(1.51±1.26)mm,respectively.And the average value and standard deviation of displacementon LR,SI and AP of CBCT was(1.27±1.18)mm,(4.08±3.75)mm,(4.25±3.82)mm,respectively.The displacement error of the two methodson three directions were less than 5 mm,and the fractional percentage were both more than 70%.Conclusion The difference of setup error for prostate cancer of KVgrade orthogonal X-ray photography and CBCT is not significant,and the adaptive method should be chosen according to the actual clinical situation.
