彩色多普勒超声对肾细胞癌的诊断价值分析

摘　　要：目的探讨彩色多普勒超声对肾细胞癌的诊断价值。方法回顾性分析63例肾细胞癌患者和30例肾良性肿瘤患者的临床资料,所有患者均接受彩色多普勒超声检查,以病理诊断结果为金标准,分析彩色多普勒超声诊断肾细胞癌的灵敏度、特异度、漏诊率及误诊率。结果肾细胞癌和肾良性肿瘤的边界情况、内部回声特征、血流信号特征比较,差异均有统计学意义(P﹤0.01);肾细胞癌和肾良性肿瘤的病灶直径比较,差异无统计学意义(P﹥0.05)。肾细胞癌患者的肾动脉收缩期峰值血流速度(Vmax)、舒张期峰值血流速度(Vmin)均明显高于肾良性肿瘤患者,肾动脉阻力指数(RI)明显低于肾良性肿瘤患者,差异均有统计学意义(P﹤0.01)。彩色多普勒超声诊断肾细胞癌的灵敏度为85.71%,特异度为76.67%,漏诊率为14.29%,误诊率为23.33%。结论肾细胞癌和肾良性肿瘤患者的彩色多普勒超声特征不同,彩色多普勒超声诊断肾细胞癌具有较高的灵敏度和特异度,特别是对血供丰富的肾脏肿瘤诊断价值更大,值得临床推广应用。Objective To investigate the clinical value of color Doppler ultrasonography in the diagnosis of renal cell carcinoma.Method The clinical profiles of 63 cases of renal cell carcinoma and 30 cases of benign renal tumor were collected and analyzed retrospectively in the study.All patients received color Doppler ultrasound examination before operation,in comparison with the gold standard of pathological findings,to determine the sensitivity,specificity,missed diagnosis rate and misdiagnosis rate using color Doppler ultrasound for diagnosis of renal cell carcinoma.Result Evident differences regarding the margin,characteristics of internal echo,and blood flow signals of renal cell carcinoma and benign renal tumor were observed,showing statistical significance(P<0.01);there was no significant difference between the diameter of renal cell carcinoma and the diameter of benign renal tumor(P>0.05).In renal cell carcinoma patients,the renal arterial blood flow Vmax and Vmin were significantly higher than those of benign renal tumor patients,and the renal artery resistance index(RI)of renal cell carcinoma was markedly lower than that in patients with benign renal tumor(P<0.01).The sensitivity of color Doppler ultrasound in differential diagnosis of renal cell carcinoma was 85.71%,the specificity was 76.67%,besides,the rate of missed diagnosis was 14.29%,and the misdiagnosis rate was 23.33%.Conclusion Renal cell carcinoma patients have significantly different color Doppler characteristics compared with benign renal tumor patients,and the color Doppler ultrasonography is highly sensitive and specific in the early diagnosis of renal cell carcinoma,and is applicable for renal tumor with abundant blood supply.
