摘 要:目的探讨基质金属蛋白酶9(MMP9)、基质金属蛋白酶组织抑制因子1(TIMP1)和趋化因子21(CCL21)在结直肠癌(CRC)患者血清中的表达水平及与预后的关系。方法回顾性分析106例CRC患者的临床资料,比较术前、术后1周及6次化疗后患者的血清MMP9、TIMP1及CCL21水平。应用Spearman相关分析法分析术前血清MMP9、TIMP1、CCL21表达水平与Duke's分期、分化程度的相关性,应用Cox比例风险回归模型分析影响CRC患者预后的因素。结果术后1周及6次化疗后CRC患者的血清MMP9、TIMP1及CCL21水平均低于术前,差异均有统计学意义(P﹤0.05);术后1周和6次化疗后CRC患者的血清MMP9、TIMP1及CCL21水平比较,差异均无统计学意义(P﹥0.05)。Spearman相关分析结果显示,Duke's分期与CRC患者的术前血清MMP9、TIMP1、CCL21水平均呈明显正相关(r=0.732、0.701、0.674,P﹤0.01),分化程度与CRC患者的术前血清MMP9、TIMP1、CCL21水平均呈明显负相关(r=-0.701、-0.728、-0.693,P﹤0.01)。Cox回归分析结果显示,术前MMP9、TIMP1及CCL21水平均是CRC患者预后的独立危险因素(P﹤0.05)。结论 MMP9、TIMP1及CCL21表达水平与CRC的恶性程度有关,可为CRC患者的预后判断提供重要的参考依据。Objective To explore the expression levels of serum matrix metalloproteinase 9(MMP9),tissue inhibitor of metalloproteinase 1(TIMP1)and chemokine 21(CCL21)in patients with colorectal cancer(CRC)and their relationship with prognosis.Method A total of 106 cases of CRC patients were enrolled in the study.The levels of serum MMP9,TIMP1 and CCL21 before surgery,1 week after surgery,and after 6 times of chemotherapy were compared.Spearman analysis was used to determine the correlation between preoperative expression level of MMP9,TIMP1,CCL21 and Duke’s staging and degree of differentiation,additionally,Cox proportional hazard regression model was utilized to analyze the factors influencing the prognosis of CRC patients.Result The expression levels of serum MMP9,TIMP1 and CCL21 at 1 week after operation and after 6 times of chemotherapy were significantly lower than those before operation(P<0.05),but there was no significant difference in the expression levels of serum MMP9,TIMP1 and CCL21 between 1 week after operation and after 6 times of chemotherapy(P>0.05).Spearman correlation analysis indicated that,Duke’s stage was positively correlated with the preoperative expression level of serum MMP9,TIMP1,and CCL21(r=0.732,0.701,0.674,P<0.01),while a negative correlation was observed for degree of differentiation(r=-0.701,-0.728,-0.693,P<0.01).The Cox regression analysis showed that,preoperative expression of MMP9,TIMP1 and CCL21 were independent risk factors for the prognosis of CRC patients(P<0.05).Conclusion The expression levels of MMP9,TIMP1,and CCL21 are significantly correlated with the degree of malignancy of CRC,and thus may be indicative for evaluating the prognosis of CRC.
