摘 要:目的探讨用脑涨落图结果指导难治性抑郁药物选择的可行性。方法选择难治性抑郁障碍患者63例,基线时行脑涨落图(EFG)检查和汉密尔顿抑郁量表(HAMD-24)评估。根据EFG提示的各递质相对功率异常情况,靶向个体化选择药物。并分别于用药第1周、2周、4周末重复EFG检测和HAMD评估,用χ~2或t检验对比各阶段递质相对功率异常率和HAMD得分与基线的差异。结果难治性抑郁障碍EFG呈多样化递质功率异常。随研究时间进展各异常递质相对功率均趋于正常化、HAMD评分递减,与基线相比均有统计学意义(P>0. 05)。依所EFG结果靶向选药的应答率远高于随机单药应答率。结论难治性抑郁障碍为脑内多受体功能异常,EFG有助于精准靶向药物选择。Objective To investigate the feasibility of encephalofluctuograph in drug selection for refractory depression.Methods A total of 63 patients with refractory depression were selected. Encephalofluctuograph( EFG) and Hamilton Depression Scale( HAMD-24) were evaluated at baseline. According to relative power abnormality of each neurotransmitter indicated by EFG,individualize drug selection. Evaluate EFG and HAMD at the 1 st,2 nd,4 th weekends respectively. The differences of relative power abnormal rate of neurotransmitters and HAMD score between each stages and baseline were compared by χ^2 or t test. Results The EFG of refractory depression was characterized by diversified transmitter power. The relative power of each abnormal transmitter gradually normalized with the progress of the study,and the HAMD score decreased,which was statistically significant compared with baseline. According to the EFG results,the response rate of targeted drug selection was much higher than the random single drug. Conclusion Refractory depressive disorder is a multi-receptor dysfunction in the brain,and EFG helps to accurately select target drugs.
