玻璃体腔内注射雷珠单抗联合激光治疗新生血管性青光眼的疗效研究

摘　　要：目的探讨采用玻璃体腔内注射雷珠单抗联合激光治疗新生血管性青光眼的临床效果。方法 61例新生血管性青光眼患者,按照治疗方法不同分为观察组(40例)和对照组(21例),对照组采用青光眼阀植入术进行治疗,观察组采用玻璃体腔注射雷珠单抗联合全视网膜激光+青光眼阀植入术进行治疗,对两组患者的治疗效果进行评估分析。结果观察组注药后3～7 d,38例新生血管全部消退,2例残留少许血管。观察组总治疗有效率高于对照组(P <0. 05);观察组术后1周、1月、3月时眼压均低于对照组(P <0. 05)。结论采用玻璃体腔内注射雷珠单抗联合全视网膜激光光凝方式治疗新生血管性青光眼,可有效对视网膜缺氧状态进行改善,抑制虹膜、房角、视网膜新生血管形成,有利于促进患者术后眼压恢复正常,对残余视功能的保护具有积极作用。Objective To investigate the clinical effect of intravitreal injection of thalidomide combined with laser in the treatment of neovascular glaucoma.Methods Sixty-one patients with neovascular glaucoma were divided into observation group(N=40)and control group(N=21)according to different treatment methods.The control group was treated with Ahmed valve implantation while the observation group was treated with vitreous cavity injection of leazuzumab combined with total retinal laser plus the Ahmed valve implantation.The therapeutic effects of the two groups were evaluated.Results The iris neovessels were regressed in 38 eases from 3 to 7 days after the intravitreal injection of lucentis and 2 cases remained a few neovessels.The effective rate in the observation group was significantly higher than that in the control group(P<0.05).The intraocular pressure of the observation group was significantly lower than that of the control group after 1 week,1 month and 3 months of operation(P<0.05).Conclusions The treatment of neovascular glaucoma by intravitreal injection of leazuzumab combined with total retinal laser photocoagulation can improve the status of retinal hypoxia and inhibit the formation of iris,cornea and retinal neovascularization.Thus,the therapy plays a positive role in the promotion of the postoperative intraocular pressure back to normal and the protection of the residual visual function.
