摘 要:目的探讨三维超声在女性盆底功能障碍性疾病(PFD)诊断中的价值。方法选取2016年12月~2017年12月我院收治的76例盆底功能障碍患者作为研究组,另选取同期健康体检的正常妇女60例作为参照组,分别经会阴二维、三维超声检测诊断,比较经会阴二维、三维超声诊断PFD的诊断效能。并采用三维超声检查比较两组患者不同时期盆膈裂孔前后径、面积、肛提肌厚度等指标的变化情况。结果三维超声诊断PFD的灵敏性、特异性及诊断符合率分别为89.47%、95.00%、91.91%,均高于二维超声诊断PFD的灵敏性(69.74%)、特异性(80.00%)及诊断符合率(74.26%),差异有统计学意义(P<0.05)。研究组治疗前的盆膈裂孔前后径、面积明显大于对照组(P<0.05);研究组治疗前与参照组的耻骨直肠肌厚度比较,差异无统计学意义(P>0.05)。治疗后,研究组在不同时期的盆膈裂孔前后径、面积小于治疗前,差异均有统计学意义(P<0.05);治疗后,耻骨直肠肌厚度与治疗前比较,差异无统计学意义(P>0.05)。结论经会阴三维超声能清晰的显示盆底解剖结构变化及盆底功能状态,提高PFD诊断的准确性,还能提供盆膈裂孔前后径、面积等信息,对患者的临床诊断治疗和康复评估等方面均有重要的应用价值。Objective To explore the value of three-dimensional ultrasound in the diagnosis of female pelvic floor dysfunction disease (PFD). Methods A total of 76 patients with pelvic floor dysfunction admitted to our hospital from December 2016 to December 2017 were selected as the study group, and 60 normal women who underwent physical examination in the same period were selected as the reference group, two-dimensional and three-dimensional perineal ultrasound were used to detect and diagnose PFD, the diagnostic efficacy of two-dimensional and three-dimensional perineal ultrasound was compared. Three-dimensional ultrasonography was used to compare the changes of anteroposterior diameter, area and levator ani muscle thickness of the pelvic and phrenic hiatus in the two groups at different stages. Results The sensitivity, specificity and diagnostic coincidence rate of three-dimensional ultrasound for PFD were 89.47%, 95.00% and 91.91% respectively, which were higher than those of two-dimensional ultrasound for PFD (69.74%, 80.00%, 74.26%), the differences were significant (P<0.05). The anteroposterior diameter and area of pelvic and phrenic hiatus before and after treatment in the study group were significantly larger than those in the control group (P<0.05). There was no significant difference in Puborectalis muscle thickness between the study groups before treatment and reference group (P>0.05). After treatment, the anteroposterior diameter and area of the pelvic and phrenic hiatus in the study group were smaller than those before treatment in different periods, the differences were statistically significant (P<0.05). After treatment, there was no significant difference in Puborectalis muscle thickness between before and after treatment (P>0.05). Conclusion Transperineal three-dimensional ultrasound can clearly show the changes of pelvic floor anatomy and pelvic floor function, improve the accuracy of PFD diagnosis, and provide information about the diameter and area of pelvic diaphragm fissure.
