叙事疗法介入造口患者的探索

摘　　要：因身体形象与身体机能的变化,造口患者面临着生理、心理、社会层面的诸多问题,亟需得到关注。在叙事疗法的实践框架下,面向同济大学附属东方医院造口患者开展小组工作。研究发现,受主流文化有关"洁净"与"污秽","健康"与"残疾"的影响,造口患者呈现出污名化的叙事。医务社工通过相关影响力问话、道具的运用、情景模拟等问题外化方法,协助造口患者将问题与个人分离,通过寻找独特经验、健康教育、同伴支持以及艺术疗法协助造口患者重写生命故事。Because of the changes of body image and function,patients with stoma are facing many problems at physiological,psychological and social levels,which need to be concerned.Under the practical framework of narrative therapy,this study conducted group social work for patients with stoma in East Hospital Affiliated to Tongji University.This study revealed that patients with stoma demonstrated stigma narration influenced by the mainstream culture on cleanliness and filth,health and disability.Medical social workers used relative influence questions,tools and scenario simulation to help patients separate problems from themselves.Unique experience,health education,peer support and art therapy were also used to help rewrite their life stories.
