摘 要:目的:系统评价循证医学理念在临床实践教学中对医学生理论学习和实践效果的影响。方法:在中国知网(CNKI)、万方医学网和PubMed数据库中,检索循证医学在我国临床实践教学中应用的文献,末次检索时间为2016年4月30日。按照纳入和排除标准进行筛选,评价文献研究质量,提取结局指标数据,采用RevMan 5.3软件进行Meta分析。结果:共纳入13篇文献,纳入文献质量不高。Meta分析结果显示,循证医学引入临床实践教学对提高本科生的理论成绩无明显影响(WMD=1.61,95%CI:-0.65~3.87,P=0.16),但能显著提高研究生的理论成绩(WMD=2.90,95%CI:0.67~5.13,P=0.01);引入循证医学能够显著提高本科生实践成绩(WMD=9.07,95%CI:8.43~9.71,P<0.01)及研究生的实践成绩(WMD=9.33,95%CI:6.67~11.98,P<0.01)。结论:循证医学理念有助于提高临床医学生的实践能力,并能显著提高研究生的理论成绩。Objective:To evaluate the effects of evidence-based medicine(EBM)systematically on medical students in clinical practice teaching in China. Methods:According to the inclusion and exclusion criteria,the related literatures about the teaching efficiency of EBM teaching method and traditional teaching method(LBL)in clinical practice teaching were searched in CNKI, Wanfang Medical Network and PubMed databases. After literature screening,quality evaluation and data extraction,all data were analyzed using Rev Man 5.3 software and heterogeneity among the included studies was assessed. Results:A total of 13 studies were included in this meta analysis.The qualities of studies were not high. The results of meta analysis showed that there was no significant difference in the scores of theoretical examination in undergraduate students between EBM group and LBL group(WMD=1.61, 95%CI:- 0.65-3.87,P=0.16),however,the scores of theoretical examination in postgraduate students in EBM group were significantly higher than those in LBL group(WMD=2.90,95%CI:0.67-5.13,P=0.01). The practical ability scores in undergraduate students(WMD=9.07,95%CI:8.43-9.71,P<0.01)and postgraduate students(WMD=9.33,95%CI:6.67-11.98,P<0.01)in EBM group were also significantly higher than those in LBL group. Conclusion:EBM teaching method can improve the clinical practice ability of clinical students,and can improve the mastery of theoretical knowledge of postgraduate students.
