摘 要:目的探讨上颌前突畸形采用正畸微种植体支抗治疗的效果及稳定性。方法选择2014年1月至2016年8月晋中市第二人民医院收治的100例上颌前突畸形患者,按抽签法分为研究组和对照组,每组50例。对照组采用传统口外弓支抗治疗,研究组采用正畸微种植体支抗治疗,随访2年,比较两组正畸治疗时间、软组织相关指标、牙齿相关指标、随访情况。结果研究组正畸治疗时间短于对照组,不良反应发生率低于对照组,差异有统计学意义(P<0.05);治疗后,两组下唇突度、上唇突度、上中切牙的唇倾度(U1-SN)均低于治疗前,上下唇突点及颏前点与眼耳平面夹角(Z角)、上下中切牙的凸度(U1-L1)、上颌第一磨牙近中牙尖点到腭平面的垂直距离(U6-PP)均高于治疗前,两组比较研究组优于对照组,差异有统计学意义(P<0.05);研究组在随访期间仅出现2例种植体松动,取出重新植入后未见松动。结论应用微种植体支抗治疗上颌前突畸形疗效显著,可缩短正畸治疗时间,有效改善患者覆覆盖关系及面型凸度,且不良反应较少,具有良好的稳定性,值得临床推广应用。Objective To investigate the efficacy and stability of orthodontic micro-implant anchorage for maxillary protrusion deformity.Methods A total of 100 patients with maxillary protrusion deformity admitted to Jinzhong Second People’s Hospital from January 2014 to August 2016 were selected,and they were divided into study group and control group by lottery,with 50 cases in each group.The control group was treated by conventional external oral arch anchorage,and the study group was treated by orthodontic microimplant anchorage.The patients were followed up for two years,and the time of orthodontic treatment,soft tissue related indicators,dental related indicators,and follow-up were compared between the two groups.Results The duration of orthodontic treatment in the study group was shorter than that in the control group,and the incidence of adverse reactions was lower than that in the control group,and the difference was statistically significant(P<0.05).After treatment,lower lip protrusion,upper lip protrusion and U1-SN of the two groups were lower than before treatment;Z angle,U1-L1 and U6-PP of the two groups were higher than before treatment,and the study group was superior to the control group in comparison,with statistically significant differences(P<0.05).Only 2 cases of implant loosening occurred in the study group during the follow-up period.Conclusions The effect of orthodontic micro-implant anchorage in the treatment of maxillary protrusion deformity is remarkable,which can shorten the time of orthodontic treatment,improve the overlaying relationship and the degree of surface convexity in patients with malocclusion.
