泪囊原发性恶性肿瘤22例临床病理学分析

摘　　要：目的分析泪囊原发性恶性肿瘤的临床病理学特点。方法对2014年1月至2017年12月22例(22只眼)泪囊恶性肿瘤的临床及病理资料进行回顾性分析。结果手术摘除肿瘤后,行病理学检查其性质。恶性上皮源性肿瘤12例(54. 5%):其中鳞状细胞癌5例(41.7%)、内翻性乳头状瘤恶变2例(16.7%)、未分化癌2例(16.7%)、基底细胞癌1例(8.33%)及腺癌1例(8.33%)及腺样囊性癌1例(8.33%)。恶性非上皮源性肿瘤10例(45.5%):其中黏膜相关性B细胞淋巴瘤5例(50.00%)、恶性黑色素瘤3例(30.00%)、泪囊血管平滑肌肉瘤1例(10.00%)和隆突性纤维肉瘤1例(10.00%)。结论泪囊恶性肿瘤多为鳞状细胞癌、黏膜相关性B细胞淋巴瘤和恶性黑色素瘤。Objective To analyze the clinicopathological features sac of primary malignant tumors of lacrimal. Methods The clinical and pathologic data of 22 eyes of 22 patients with primary malignant tumors from lacrimal sac of Jan. 2014 to Dec. 2017 were analyzed retrospectively. Results All cases received pathological examination of tumors after surgical excision. Among 12 cases(54. 50%) of epithelial malignant tumor: there were squamous cell carcinoma in 5 cases ( 41. 70%), inverted papilloma with malignant transfonnation in 2 cases( 16. 70%), undifferentiated carcinoma in 2 cases( 16. 70%),basal cell carcinoma in 1 case( 8. 33%), adenocarcinoma in 1 case( 8. 33%) and adenoid cystic carcinoma in 1 case (8. 33%). Among 10 cases(45. 50%) with nonepithelial malignant tumors: there were B-cell lymphoma of mucosa-associated lymphoid tissue in 5 cases ( 50. 00%), malignant melanoma in 3 cases ( 30. 00%), lacrimal sac leiomyosarcoma in 1 case ( 10. 00%) and dermatofibrosarcoma protuberans in 1 case (10. 00%). Conclusion Most malignant lacrimal tumors are squamous cell carcinoma, B-cell lymphoma of mucosa-associated lymphoid tissue and malignant melanoma.
