白内障术后视网膜脱离的危险因素分析

摘　　要：目的分析白内障手术人工晶状体植入术后出现视网膜脱离(RD)的危险因素。方法回顾性分析2010年6月至2016年5月本院收治的白内障3 548例(3 548眼)的临床资料,随访时间2年。分析术中医源性因素、年龄、眼轴长度及近视等因素与术后视网视脱离的关系。结果术后发生RD 36例,占所有白内障手术的1.01%;男性比例较高;眼轴小于24 mm的RD患者发生率较低,而眼轴为27-28 mm的RD患者发生率高达3.59%;白内障手术后RD发生率在50?59岁人群中发生率最高,平均年龄为(58.56 ±9.9)岁,随着年龄的增加RD发生率显著降低。术中并发晶状体后囊破裂、后发性白内障行激光后囊切开术与视网膜脱离密切相关。结论白内障术后视网膜脱离与性别、年龄、眼轴长度、高度近视、术中晶状体后囊破裂及后发性白内障行激光后囊切开术密切相关。Objective To analyze risk factors of retinal detachment after cataract surgery combined with intraocular lens implantation. Methods The clinical data of 3 548 eyes of 3 548 cases of cataract from Jun. 2010 to May 2016 were analyzed retrospectively. Patients were followed up for 24 months. The relationship of iatrogenic factors, age, axial length, myopia, etc with retinal detachniant was analyzed. Results After cataract surgery retinal detachment occurred in 36 patients, accounting for 1.01%, males were more than females. In addition, the incidence of retinal detachment with axial length < 24 mm was lower. And the incidence of patients with 27-28 mm of axial length was 3. 59 %. The incidence of retinal detachment after catciract surgejy was highest among people aged 50-59 years, the average age was ( 58. 56 ±9.9) years. The incidence of retinal detachment decreased significantly with age increase. Retinal detachment was closely related to posterior capsule rupture during surgery and postoperative cataract treated with capsulotomy. Conclusion Retinal detachment is closely related to gender, age, axial length, high myopia, posterior capsule rupture during surgery and postoperative cataract treated with capsulotomy.
