翼状胬肉术后两种糖皮质激素滴眼液效果比较

摘　　要：目的比较翼状鹫肉切除术后应用两种糖皮质激素滴眼液的效果。方法回顾性分析2016年3月至2017年2月在大庆油田总医院治疗的原发性翼状窗肉90例(90眼)的临床资料,均行翼状閤肉切除联合自体角膜缘干细胞移植术,术后用药随机分为A组(对照组)、B组(典必殊组)和C组(氟米龙组),每组各30例,进行效果比较.结果3组患者术前一般情况差异无统计学意义(P>0. 05),术后术眼异物感持续时间:三组依次为(11. 43 ±3. 43) d、 8.87 ±3. 15) d和(8.87 ±3.15)d,差异有统计学意义(F = 6. 255,P<0.05);移植片水肿时间:三组依次为(22. 53 ±8.12) d、(16. 13±5.53)d和(16.67±6.06)d,差异有统计学意义(F =8.514,P<0.05);角膜上皮修复时间:三组依次为(3.60±0.77)d、(4.60±0.86)d 和(3. 77 ±0.63)d,差异有统计学意义(F=15.054,P<0.05);基础泪液分泌试验及泪膜破裂时间:组间及各组不同时间相比差异均有统计学意义(P<0. 05),翼状歸肉复发率:三组依次为13.33%、3. 33%和3. 33%(X^2 = 14. 320, P < 0. 05), B组有1例发生高眼压.结论氟米龙联合0.1%爱丽滴眼液应用于翼状鹫肉切除术后效果较好。Objective To compare the efficacy between two kinds of glucocorticoid drops after pterygium surgery. Methods The data of 90 eyes of 90 patients with primary pterygium who received pterygium excision combined with corneal limbal stem cell transplantation from Mar. 2016 to Feb. 2017 in Daqing Oil Field General Hospital were analyzed retrospectively. The patients were divided into three groups randomly, each group of 30 cases. Group A did not use glucocorticoid drops as control group, group B received application of TobraDex drops and group C received application of Flunietholon drops. The efficacy were compared among the 3 groups. Results The difference was not statistically significant in gender, age, pterygium size or growth time among three groups (P >0.05). Postoperative duration of unwell symptoms were (11.43 ±3.43) d in group A,(8. 87 ± 3. 15) cl in group B, and (8. 87 ± 3. 15 ) d in group C, the difference was statistically significant among the three groups ( F =6. 255 , P < 0. 05 ). The period of graft edema was (22. 53 ± 8. 12) d in group A,( 16. 13 ±5.53)(1 in group B and ( 16. 67 ± 6. 06) d in group C, and the difference was statistically significant ( F = 8. 514, P < 0. 05 ). The corneal epithelial healing time was (3. 60 ± 0. 77)(1 in group A,(4. 60 ± 0. 86) d in group B and (3.77±0. 63) d in group C, and the difference was statistically significant ( F = 15. 054, P <0. 05 ). The difference of schimier I test ( SIT) and tear break up time ( BUT) were statistically significant among groups and in each group at different times (P<0.05). The recurrence rates was 13. 33% in group A, 3. 33% in group B, and 3. 33 % in group C (X^2=14. 320, P < 0. 05 ). Conclusion The application of flunietholon eye drops combined 0. 1% Hialid drops after pterygiuin surgery is more effective.
