摘 要:目的探讨DCE-MRI直方图纹理参数在乳腺纤维瘤和浸润性导管癌的鉴别诊断价值。方法回顾性分析经病理证实的51例乳腺病变(乳腺纤维瘤21例,浸润性导管癌30例),所有患者在活检或手术前均行DCE-MRI检查。手动勾画病灶ROI,分别对每个ROI提取DCE定量参数容积转移常数(K^trans)、速率常数(Kep)和血管外细胞外间隙容积比(Ve)的直方图纹理参数(能量、熵、偏态、峰态、平均值、中位数),通过绘制ROC曲线,得到每个参数的AUC。结果除过DCE定量参数Kep的峰态,Ve的能量、熵和峰态这些纹理参数在2组间差异无统计学差异,其他直方图参数均可对2组进行鉴别,2组间有统计学差异(P<0.05);K^trans 、Kep和 Ve的平均值的AUC最高,分别为0.79、0.92、0.73。结论乳腺纤维瘤和浸润性导管癌的DCE定量参数的直方图纹理特征具有显著差异,DCE-MRI直方图纹理分析可对乳腺纤维瘤和浸润性导管癌做出准确的鉴别诊断,有助于临床治疗方式的选择。Objective To assess the diagnostic value of texture analysis of DCE-MRI in differential of breast fibroadenoma ( FA) and invasive ductal carcinoma (IDC).Methods The MRI data of 51 patients (21 breast FA,30 IDC) confirmed by surgical pathology were retrospectively studied.6 texture features of the DCE metric parameters, including forward-volume transfer contrant ( K^rans) x reverse volume transfer constant(Kep ) and extravascular extracellular space volume fraction ( Ve), were generated from manually selected ROI of the lesions. ROC curve of texture parameters were used in differential diagnosis of FA and IDC. Results Significant differences were found between FA and IDC for all DCE-MRI metrics except for Kep kurtosis, Ve energy,entropy,and kurtosis.The mean value of K^trans、Kep and Ve were 0.79、0.92、0.73, respectively.Conclusion The histogram texture characteristics on the DCE quantitative parameter of FA and IDC have significantly differences. Histogram texture analysis of DCE-MRI demonstrates the ability of differentiating FA from IDC, and provides reliable results for clinical diagnosis, which is helpful for the choice of clinical treatment.
