甲状腺乳头状癌合并桥本甲状腺炎的超声造影与BRAF^(V600E)突变分析

摘　　要：目的探讨合并桥本甲状腺炎(HT)对甲状腺乳头状癌(PTC)超声造影(CEUS)与BRAF^(V600E)突变的影响。方法收集我院手术病理证实为PTC患者122例。按照是否合并HT分为HT组和非HT组。比较两组间年龄、性别、病灶最大径、超声表现、CEUS以及BRAF^(V600E)突变率等差异。结果 HT组50例,非HT组72例,两组间发生BRAF^(V600E)突变率差异有统计学意义(P=0.003);两组间女性患者例数差异有统计学意义(P=0.007);两组间患者年龄(P=0.219)、病灶最大径(P=0.096)、超声表现、CEUS特征及PI (P=0.664)、△PI (P=0.176)等参数差异均无统计学意义。结论 PTC合并HT更常见于女性;合并HT者BRAF^(V600E)突变率低于不合并HT者;是否合并HT对PTC的血流灌注无影响。Objective To investigate the influence of accompanied Hashimoto′s thyroiditis(HT)on the CEUS and BRAFV600E mutation of papillary thyroid carcinoma(PTC).Methods 122 patients confirmed by surgical pathology as PTC were enrolled in the study.According to the status of accompanied HT,the patients were divided into two groups:the HT group and the non-HT group.To compare the age,sex,maximum length of lesion,ultlasound feature,CEUS and BRAFV600E mutation rate between the two groups.Results 50 patients accompanied with HT and 72 patients did not.The difference of BRAFV600E mutation between the two groups was statistically significant(P=0.003).The female patient ratio between the two groups was statistically significant(P=0.007).There was no significant difference in age(P=0.219),maximum length of lesion(P=0.096),ultrasound features and CEUS feature and PI(P=0.664)and △PI(P=0.176).Conclusions PTC accompanied with HT is more common in women.The mutation rate of BRAFV600E in PTC with HT was significantly lower than in PTC without HT.Weather or not accompanied with HT has no effect on PTC perfusion.
