摘　　要：目的:探讨针刺联合安神脐贴对脑梗死后伴有失眠患者的促眠作用及其对血浆5-HT、NE、DA的影响。方法:收集我院收治的脑梗死后伴有失眠患者88例,依据随机数字表法随机分为对照组和治疗组,各44例,对照组予以西医常规治疗;治疗组予以针刺联合安神脐贴进行治疗,两组患者均治疗4周。比较两组患者临床疗效、睡眠质量及神经功能缺损改善情况及血浆5-HT、NE、DA表达水平。结果:治疗后,临床检测统计可知,治疗组临床总有效率为95.46%,对照组临床总有效率为70.46%,治疗组较对照组改善明显,两组结果具有显著差异(P<0.05);经治疗,与对照组患者相比,治疗组患者睡眠质量及神经功能缺损评分改善更明显(P<0.05);经治疗,与对照组患者相比,治疗组患者血浆5-HT、NE、DA表达水平升高更显著,差异具有统计学意义(P<0.05)。结论:针刺联合安神脐贴对脑梗死后伴有失眠的治疗取得较为满意的临床疗效,具有镇静安神、养血宁心之功效,能够改善患者临床症状,提高患者睡眠质量,发挥了中医外治疗法优势,值得临床推广应用。Objective:To investigate the sleep promotion of acupuncture combined with Anshen Navel Plaster in patients with insomnia after cerebral infarction,and its effect on plasma 5-HT,NE,DA. Methods:88 patients with insomnia after cerebral infarction in our hospital were randomly divided into control group and treatment group according to random number table method,44 cases in each group. The control group was treated with routine Western medicine,the treatment group was treated with Anshen Navel Plaster.Both groups were treated for 4 weeks. The clinical efficacy,sleep quality,neurological impairment and plasma 5-HT,NE,DA levels were compared between the two groups. Results:After treatment,clinical test statistics showed that the total effective rate of the treatment group and the control group were 95.46% and70.46%,respectively. Compared with the control group,the treatment group improved significantly,and the results of the two groups were significantly different(P<0.05). After treatment,the improvement of sleep quality and neurological deficit score in the treatment group was significantly better than that in the control group(P<0.05). After treatment,compared with the control group,the plasma 5-HT,NE,DA expression level in the treatment group was significantly higher than that in the control group(P<0.05). Conclusion:Acupuncture combined with Anshen Navel Plaster can obtain a satisfactory clinical curative effect in the patients with insomnia after cerebral infarction,has the efficacy of tranquilizing mind,allaying excitement,nourishing blood and calming the heart,can improve the clinical symptoms and the sleep-quality of patients.The treatment method plays the advantage of TCM external treatment method and is worthy of clinical popularization and application.
