急诊胃镜治疗食管胃静脉曲张破裂出血的应用价值

摘　　要：目的研究急诊胃镜治疗食管胃静脉曲张破裂出血的应用价值。方法选取我院2016年12月至2018年8月收治的51例食管胃静脉曲张破裂出血患者,所有患者均采用急诊胃镜治疗,观察其临床治疗效果。结果 51例患者中,经急诊胃镜止血治疗后均出血缓解并出院,急诊止血有效率达到100.00%,早期再出血率为0.00%;患者均未出现术后严重并发症。食管曲张静脉直径、红色征发生率手术前后比较有统计学差异(P <0.05);门静脉高压性胃病发生率手术前后比较无统计学差异(P>0.05)。结论在食管胃静脉曲张破裂出血的治疗中,急诊内镜能够及时有效地控制患者的出血状况,同时患者术后并发症和早期再出血发生率极低,保障治疗的有效性和安全性,适宜广泛应用于基层医院急诊治疗中。Objective To study the application value of emergency gastroscopy in the treatment of esophageal and gastric variceal bleeding. Methods 51 cases of patients with esophageal and gastric variceal bleeding admitted to our hospital from December 2016 to August 2018 were selected and treated with emergency gastroscopy. The clinical treatment effect was observed. Results Among the 51 patients, the bleeding was relieved and discharged after emergency gastroscopic hemostasis, the effective rate of emergency hemostasis was100.00% and the rate of early rebleeding was 0.00%. No serious postoperative complications occurred. Statistical difference was found in the diameter of varicose vein and incidence of red sign before and after operation(P<0.05). No statistical difference was found in the incidence of portal hypertensive gastropathy before and after operation(P>0.05). Conclusions In the treatment of esophageal and gastric variceal bleeding, emergency gastroscopy can effectively and timely control the bleeding of the patients, and the incidences of postoperative complications and early rebleeding are very low. The efficacy and safety of the treatment can be guaranteed, so it is suitable to be widely used in the emergency treatment of primary hospitals.
